Objective To explore the application effect of multi-component home-based exercise in elderly patients with osteoporosis and frailty. Methods A total of 52 elderly patients with osteoporosis and frailty were recruited from the osteoporosis clinic of a tertiary hospital in Nanjing,Jiangsu Province from March to August 2022 using the convenience sampling method. Subjects were randomly divided into an experimental group and a control group with 26 cases in each group by the random number table. The experimental group was given the home-based exercise program,while the control group maintained the daily lifestyle. Short Physical Performance Battery(SPPB),grip strength,Frail Scale and Chinese Osteoporosis-targeted Quality of Life Questionnaire(COQOL) of the 2 groups were compared at 12 weeks and 24 weeks,and bone mineral density(BMD) was analyzed after 24-week intervention. Results The scores in the experimental group had significant statistical differences in SPPB,grip strength,Frail Scale and COQOL compared with those in the control group after 12-week and 24-week intervention(P<0.05). The BMD of total hip of the experimental group was significantly higher than it in the control group(P<0.05). Conclusion The home-based exercise program can help the elderly with osteoporosis and frailty to improve the physical function and upper body strength,improve the frailty state,and enhance the quality of life,and it may be one of the effective intervention methods to prevent and reverse the frailty state of osteoporosis patients.
As one of the subtypes of frailty,cognitive frailty is reversible and can effectively predict negative health events in older adults such as dementia,disability,and functional decline. This article introduces the basic information about the universal cognitive frailty risk prediction model and disease-specific cognitive frailty risk prediction model,analyzes and compares the research methods and inclusion of risk factors in the cognitive frailty risk prediction model,and provides insights into the future,in order to provide references for medical and nursing staff to early identify the risk of cognitive frailty in older adults and provide effective intervention measures.
As a special care unit,the unit of acute care for the elderly(ACE) focused on functional maintaining of elderly patients during hospitalization to recover rapidly and return to the family as soon as possible. This paper summarized the origin,development,key elements,working procedures and application effects of the ACE unit,and suggested the development direction of ACE unit according to Chinese national conditions,which can be used as a reference for nursing clinical practice.

Methods We systematically searched for the evidence on prevention and management of the skin adverse reactions in patients with targeted tumor therapy in BMJ best practice,UpToDate clinical decision system,Joanna Briggs Institute,Guidelines International Network,National Institute for Health and Clinical Excellence,Scottish Intercollegiate Guidelines Network,Yi Maitong Guidelines Network,National Guideline Clearinghouse,the Agency for Healthcare Research and Quality,Registered Nurses’ Association of Ontario,World Oncology Network,National Comprehensive Cancer Network,Lung Cancer Committee of China Anti-Cancer Association,Chinese Anti-Cancer Association Oncologic Nursing Comittee,Cochrane Library,Embase,CHINAL,Web of Science,OVID,PubMed,SinoMed,CNKI,WanFang database,VIP database to collect the literature,including clinical guidelines,evidence summaries,best practice information sheets,recommended practice,systematic reviews and original research. The retrieval period was from January 1,2011 to October 3,2021.There were 2 researchers evaluating the quality of the literature and extracting the data. Results 15 articles were selected,including 3 guidelines,4 evidence summaries,1 systematic reviews,3 randomized controlled trials,1 class experiment and 3 expert consensuses. 19 pieces of best evidence were formed from 6 aspects,including skin evaluation,risk factors,drug therapy,skin care,skin adverse reaction management and health education. Conclusion This study summarizes the best evidence on management and prevention of skin adverse reactions in patients with targeted tumor therapy,and provides evidence-based evidence for clinical workers and managers. It is suggested that clinical staff should combine clinical context,fully assess patient’s skin condition,do a good job of skin health education,examination and care to prevent the occurrence of skin adverse reactions.
Objective To search,evaluate and summarize the best evidence of self volume-management for patients with chronic heart failure(CHF) based on the situation-specific theory of heart failure self-care. Methods The computer decision support systems,guideline websites,databases were systematically searched to collect the literature related to self volume-management in CHF patients. The retrieval period is from the establishment of databases to May 4th,2021. 2 researchers assessed the quality of the included literature independently and extracted the evidence. Results A total of 9 articles were included,including 7 clinical practice guidelines and 2 expert consensuses. A total of 13 pieces of evidence have been obtained from 3 aspects including self volume-management maintenance,symptom perception of volume overload,and volume overload management. Conclusion The best evidence should be used to help CHF patients maintain a balance of volume status,recognize changes in volume status as soon as possible,and learn some skills to respond appropriately to volume overload.
Fluid management is a prevention and management strategy of heart disease. The aim of this strategy is to prevent and control fluid retention in patients with heart failure,thereby reducing the re-hospitalization rate and mortality,and improving their quality of life. This paper summarizes the specific content and development form of liquid management,and points out the defects and improvement direction of current liquid management research,aiming to arouse the attention of nurses on fluid management of patients with heart failure,and to explore a suitable fluid management model for them.
Objective To evaluate clinical application effects of standardized management of inter-hospital referral process. Methods The process of inter-hospital referral and the checklist of handover were formulated through literature reviews,expert discussions and consultations. The process and the checklist were adjusted by modified Delphi method,and were applied into the clinical practice for 2 months. The impact on the duration of inter-hospital referral and the incidence of adverse events was observed. Results After the clinical application,the rate of the communication between referral hospitals before the transferring has reached 100%;the completion rate of checklists was 96.15%;the length of time on handover fell from(14.44±3.52)mins to(9.33±1.83)mins;the incidence of adverse events decreased from 21.05% to 3.84%;the differences were statistically significant(P<0.01,P<0.05). Conclusion The process and the checklist of inter-hospital referral are scientific and practical. They standardized the process,improved the work efficiency and reduced the incidence of adverse events,which can be applied to clinical practice and improve patient safety.
