    Application research of ultrasound oblique axis plane guidance technique in PICC puncture

    Objective To explore the application efficacy and safety of oblique ultrasound-guided techniques in PICC puncture,in order to provide guidance and references for clinical application. Methods Through convenient sampling,654 patients from a tertiary A hospital in Zhejiang Province from March to December 2022 were selected as the study subjects. The random numbers were generated through Excel table functions and they were randomly grouped into 3 groups:A,B,and C. The ultrasound short axis method,long axis method,and oblique axis method were employed to guide PICC puncture catheterization,respectively. The success rate of PICC puncture,the number of subcutaneous adjustments of the puncture needle,puncture time,and the occurrence of puncture complications (such as hematoma,puncture of the posterior wall of blood vessels,accidental injury to arteries,and accidental injury to nerves) were recorded during the catheterization process in 3 groups. Results A total of 654 patients completed the study,including 215 in group A,219 in group B,and 220 in group C. The success rate of first-time puncture in the group C(86.36%) was higher than that in group A(73.95%) and group B(63.93%),and there was a statistically significant difference among 3 groups(P<0.001). The subcutaneous adjustment frequency of the puncture needle was 1(1,1) in group C,1(1,2) in group A,and 1(1,2) in group B. The difference between 3 groups was statistically significant(P<0.001);the puncture time of group C was shorter than that of group A and group B,and the difference was statistically significant(P<0.001). There was a statistically significant difference in the puncture time between 3 groups(P<0.017);the pairwise comparison of the number of subcutaneous needle adjustments and the success rate of a puncture between 3 groups showed that there was a statistical difference between group C and group A,and between group C and group B(P<0.017),while there was no statistical difference between group A and group B(P>0.017).There was statistical significance(P<0.05) among 3 groups in terms of complications such as accidental nerve injury and puncture of the contralateral vascular wall by puncture needle,but there was no statistical significance in terms of accidental arterial injury and hematoma occurrence among 3 groups. Conclusion Compared with the short axis approach and the long axis approach,the ultrasound oblique axis approach guided PICC puncture has statistical differences in the success rate of a puncture and the incidence of puncture complications,etc. It is recommended to use the ultrasound oblique axis approach during PICC puncture.

    A study on the application of blunt separation method in midline catheter intubation in elderly patients with coagulation dysfunction

    Objective To explore the efficacy of blunt separation method in midline catheter intubation in elderly patients with coagulation dysfunction. Methods A total of 80 elderly patients with coagulation dysfunction were selected in the convenience sampling method from October 2022 to April 2023 in our hospital,and they were randomly divided into an experimental(blunt) group and a control(routine) group,with 40 patients in each group. The differences in the degree of bleeding and exudation at the puncture site,the pain score and the incidence of complications(including bleeding and exudation,phlebitis,symptomatic catheter-related thrombus,catheter blockage,catheter pulling-off) were compared between 2 groups. Results In the experimental group,the degree of bleeding and exudation at the puncture point immediately after the operation,degree of bleeding and exudation at the puncture point 24 hours after the operation,pain score 1 day after the catheterization,pain score 3 days after the catheterization,incidence of bleeding and exudation,total incidence of complications and maintenance times were significantly lower than these in the control group(P<0.05). In terms of the pain score immediately after the operation,pain score 5 days after the operation,incidence of phlebitis,incidence of symptomatic catheter-related thrombosis,incidence of catheter blockage,incidence of catheter pulling-off,incidence of catheter related skin injury,incidence of unplanned extubation,success rate of one-time sheath delivery and the indwelling time,the differences between the experimental group and control group were not significant(P<0.05). Conclusion The application of blunt separation method in midline catheter indwelling can significantly reduce the incidence and degree of bleeding at the puncture point,decrease the maintenance times and relieve the pain in elderly patients with coagulation dysfunction.

    Objective To investigate the appropriate venous access for obese patients undergoing metabolic and bariatric surgery by comparing the clinical outcomes of different lengths of peripheral intravenous catheters. Methods Inpatients who underwent bariatric surgery in a tertiary hospital in Zhejiang from August 2022 to December 2022 were selected as the study population using a fixed-point continuous convenience sampling method. A stratified block randomisation method was used to divide the group into an experimental group 1(mini-midline catheters),an experimental group 2(midline catheters) and a control group(short peripheral intravenous catheters,Short PIVCs). The incidence of catheter-related complications,the rate of extubation due to complications,the duration of catheter retention,the time to first catheter-related complication were compared in the 3 groups. Results A total of 186 patients were included,with 62 patients in each group. The overall incidence of catheter-related complications in experimental group 1,experimental group 2,and control group were 25.81%,8.06%,and 58.06%. The extubation rates due to complications were 19.35%,4.84%,and 41.94%,and the duration of catheter retention was 7.00(6.00,7.00) d,7.00(6.00,7.00) d,6.00(3.00,6.25) d. The differences were statistically different(P<0.05) when comparing the 3 groups. Among them,the differences in the overall incidence of catheter-related complications and the rate of extubation due to complications were statistically significant when comparing experimental group 1 with the control group,experimental group 2 with the control group,and experimental group 1 with experimental group 2(P<0.017);the duration of catheter retention in both experimental group 1 and experimental group 2 were higher than it in the control group,and the differences were statistically different(P<0.017). Conclusion The complication rate of mini-midline catheters and midline catheters is lower than that of short ones,and the indwelling time is consistent with the perioperative period of metabolic and bariatric surgery,which is suitable for use in patients undergoing metabolic and bariatric surgery.

    Objective To search,evaluate,and summarize the best evidence of insertion and maintenance of mini-midline in adult patients,in order to provide a solid evidence-based foundation for establishing clinical standardized and safe utilization of mini-midline. Methods We systematically searched UpToDate,BMJ Best Practice,Guideline International Network,National Guideline Clearinghouse,Joanna Briggs Institute,National Institute for Health and Care Excellence,Centers for Disease Control and Prevention,Scottish Intercollegiate Guidelines Network,Canadian Medical Association:Clinical Practice Guideline,Registered Nurses’ Association of Ontario,Intravenous Nurses Society,Cochrane Library,PubMed,CINAHL,Embase,Web of Science,Yimaitong,China National Knowledge Infrastructure,Wanfang Database,and China Biomedical Literature Database. It includes clinical practice guidelines,expert consensuses,evidence summaries,systematic reviews,meta-analyses,and best practice on mini-midline insertion and maintenance. The search period covered January 2013 to May 2023. 2 researchers independently evaluated the literature quality and extracted the literature that met the standards. The evidence was graded using the JBI evidence pre-grading system(2014),and the extracted evidence was classified and summarized by themes. Results A total of 10 pieces of the literature were involved,including 6 guidelines,2 expert consensuses,and 2 systematic reviews. This study summarized 20 pieces of best evidence in relation to the following 4 themes:indications and contraindications for mini-midline,catheter insertion,catheter maintenance and catheter removal. Conclusion The study comprehensively and scientifically summarizes the best evidence regarding the insertion and maintenance of mini-midline. We recommend that clinical practitioners integrate this evidence into their practice,while considering individual patient preferences and medical contexts. This approach,in line with the principle of individualization,will contribute to enhancing the standardization and safety of mini-midline use in clinical settings.

    Common incidence of catheter-related venous thrombosis between midline catheter and peripherally inserted central catheter:a Meta-analysis

    Objective To compare the incidence of catheter-related venous thrombosis between midline catheters and peripherally inserted central catheters. Methods Randomized controlled trials and cohort studies on the incidence of venous thrombosis associated with midline catheters and PICC catheters were searched from CNKI,Wanfang database,VIP database,Web of Science,PubMed,Embase and Cochrane Library from inception to December 31,2022. Review Manager 5.4 software and Stata 14.0 software were used to analyse and describe the outcome indicators. Results A total of 16 studies were included,including 12 cohort studies and 4 randomized controlled studies,with 21853 subjects. The results of the Meta-analysis showed that the incidence of catheter-related venous thrombosis per thousand catheter days of midline catheters was statistically significant compared with PICC[RR=2.74,95%CI(1.21,6.21),P=0.016]. There was no significant difference in the incidence of catheter-related venous thrombosis compared with PICC[RR=0.85,95%CI=(0.70,1.03),P=0.101]. In the subgroups,the incidence of superficial vein thrombosis in the midline catheter was significantly different from that in the PICC[RR=2.36,95%CI=(1.56,3.58),P<0.001]. Conclusion The current evidence shows that the incidence rate of catheter-related venous thrombosis per thousand catheter days and superficial vein thrombosis was higher for midline catheters than PICCs. Therefore,in clinical practice,vascular access devices should be selected reasonably,and the occurrence and development of catheter-related superficial venous thrombosis should be paid attention to,and clinical screening should be effectively carried out on the basis of a full evaluation.

    Construction of nursing quality evaluation indicators in perioperative period of heart transplantation

    Objective To construct quality evaluation indicators for perioperative nursing in heart transplantation,and to provide standard and professional quantitative bases for monitoring and management of perioperative nursing quality. Methods This study was conducted based on the frame work of the three-dimensional “structure-process-outcome” quality model,using literature review,Delphi method and analytic hierarchy to determine the content of the indicators,and the weight of each index. Results A total of 22 experts from 14 qualified heart transplantation hospitals were included,and a total of 2 rounds of consultations were conducted. The effective recovery rates of 2 rounds of expert consultation questionnaires were 100%. The authority coefficients were 0.817. The variation coefficients of each item ranged from 0.025~0.169 and 0.039~0.157. The Kendall harmony coefficients were 0.126 and 0.225(P<0.001). The final evaluation indicators for perioperative nursing quality in heart transplantation included 3 first-level indicators,12 second-lever indicators and 59 third-level indicators. Conclusion The evaluation indicators of perioperative nursing quality in heart transplantation was scientific,comprehensive and specialized,which can provide references for the evaluation of perioperative nursing quality in heart transplantation.

    Objective To construct a domain knowledge graph of dementia care,so as to provide the foundation and guarantee for the next intelligent application based on the knowledge graph. Methods A top-down approach was adopted to construct a domain knowledge graph of dementia care. Firstly,the ontology concept is constructed from the top level,namely the schema layer of knowledge graph. Then,instances are filled,and knowledge extraction is carried out from the existing data sources,and the extracted entities and relationships are filled into the pattern layer ontology database to complete the data layer construction of the knowledge graph. Finally,the “entity relationship entity” triplet data was input into the Neo4j graph database for storage. Results In this study,the personalized care plan set of 1 012 dementia cases was used as the corpus to construct a domain knowledge graph of dementia care. The knowledge graph takes people with dementia as the core,and unfolds,one by one,around basic characteristics,care problems,and care plans in a standardized “entity-relationship-entity” triplet format,forming a large knowledge network,which contains a total of 1 522 specific dementia care knowledge entities and 8 kinds of inter-entity relationships. Conclusion The domain knowledge graph of dementia care constructed in this study clearly and intuitively shows the global pedigree and logical path of knowledge,which provides an efficient and intelligent basic guarantee for the browsing,retrieval and application of dementia care knowledge,so as to realize personalized and intelligent management of people with dementia,break through the bottleneck of lack of professionals,improve the health outcomes of people with dementia,promote the implementation of inclusive pension services,and promote healthy aging.

    The development of a health-promoting lifestyle scale for patients with polycystic ovary syndrome and the test of its reliability and validity

    Objective To develop a health-promoting lifestyle scale for women with polycystic ovary syndrome (PCOS),and to test its reliability and validity,and preliminarily apply it. Methods Based on the Pender health promotion model,the item pool of the scale was constructed through literature research,semi-structured interviews and group discussions. After 2 rounds of Delphi expert consultation and pre-survey,the initial scale was formed. From April to July 2022,316 patients with PCOS in the health management center,reproductive medicine center and endocrinology department of a tertiary hospital in Nanjing were selected for item analysis,exploratory factor analysis and reliability test,respectively. From August to October 2022,358 PCOS patients were selected for confirmatory factor analysis. From November 2022 to February 2013,294 PCOS patients were selected,and the scale was used to investigate the status of health-promoting lifestyle in PCOS patients. Results The health-promoting lifestyle scale for PCOS patients included 5 dimensions and 33 items. The total content validity index of the scale was 0.942,and the content validity index of each item was 0.810-1.000. 5 common factors were extracted by 2 exploratory factor analyses,and the cumulative variance contribution rate was 62.399%. Confirmatory factor analysis showed that the model fit was good. The Cronbach’s α coefficient of the total scale was 0.930;the split-half reliability was 0.842;the test-retest reliability was 0.888. The preliminary application results showed that the total score of health-promoting lifestyle in PCOS patients was(96.925±14.273),and the average score of items was (2.937±0.433),which was at a medium level. Conclusion The health-promoting lifestyle scale for PCOS patients has good reliability and validity,which can be used as a tool for medical staff to assess the level of health-promoting lifestyle of PCOS patients,and can help nurses to quickly identify the level and dimensions of health-promoting lifestyle of patients,so as to formulate individualized precise health management plans.

    A multicenter survey on nurses’ knowledge and practice of children’s sleep management in pediatric ICU

    Objective To investigate the current status of nurses’ knowledge,attitude and practice regarding sleep management of critically ill children in pediatric ICU,and to analyze its impact factors. Methods A self-designed questionnaire on general information and a questionnaire on knowledge and practical behaviors of pediatric ICU nurses on child’s sleep management were used. In March 2023,902 pediatric ICU nurses from 24 hospitals in China were surveyed using a convenient sampling method,and the impact factors were analyzed using multiple stepwise linear regression. Results 893 valid questionnaires were collected and the recovery rate of valid questionnaires was 99.00%. Nurses in pediatric ICU scored(33.71±7.76) in knowledge dimension,(37.38±4.86) in attitude dimension and(80.60±16.78) in practice dimension,with a total score of(151.78±24.27). The scores of knowledge and attitude,knowledge and practice,attitude and practice are all positively correlated (r=0.393,P<0.001;r=0.495,P<0.001;r=0.320,P<0.001). The results of multiple stepwise linear regression analysis showed that gender,region,whether they had received sleep management training were the influencing factors of pediatric ICU nurses’ total score of knowledge,attitude and practice towards children’s sleep management(P<0.05). Conclusion Nurses in pediatric ICU are positive about sleep management for critically ill children,but their knowledge and practice levels need to improve. Nursing managers should strengthen the theoretical knowledge and practical behavioral training of pediatric ICU nurses on child sleep management,develop scientific sleep management plans,and guide nurses to make reasonable evaluation and interventions to improve children’s sleep quality.

    Status and influencing factors of the discharge readiness of fathers of preterm infants in NICU

    Objective To understand the discharge readiness of fathers of preterm infants in the Neonatal Intensive Care Unit(NICU),analyze its influencing factors,and provide a basis for formulating individualized discharge health education plans and continuous care plans. Methods A convenience sampling method was employed to select fathers of preterm infants admitted to the NICU in a tertiary A hospital in Zhejiang Province from March 2022 to June 2022 as the subjects of the study. A general information questionnaire and the Discharge Readiness-Parent Version scale were used for the survey. Multiple linear regression analysis was applied to analyze the factors influencing the discharge readiness of fathers of NICU preterm infants. Results A total of 186 fathers of NICU preterm infants were included in the study. The total score of the Discharge Readiness-Parent Version scale was(179.97±28.20) points;the standardized score was(7.82±1.16) points;33.3% of the fathers of preterm infants had a standardized score of <7 points. Multiple linear regression analysis results showed that the factors influencing the discharge readiness of fathers of NICU preterm infants included the gestational age at birth,birth weight,length of hospital stay,distance to access healthcare service,as well as the fathers’ age,annual family income,and number of children in the family. Conclusion The discharge readiness of fathers of NICU preterm infants is at a moderate level. Nursing staff need to pay attention to the discharge readiness of fathers of preterm infants who have lower gestational age at birth,lower birth weight,longer hospital stays,longer distance to healthcare service,and who are younger,have lower annual family income,and are having their first child. Individualized discharge health education plans and continuous care plans should be formulated for them to improve their caregiving knowledge and skills.

    To summarize the nursing points of enterostomy among 6 children with small intestinal transplantation,which could provide references for clinical nursing. Key points:to set up a multidisciplinary team and develop an observation plan about enterostomy;to monitor the blood circulation of enterostomy closely,so as to identify the vascular complications as soon as possible;to closely observe the excrement of enterostomy and being vigilant for rejection reaction;to select reasonable nursing supplies based on the situation of enterostomy;to treat the abdominal incision fluid leakage timely,and pay attention to protect the skin around the enterostomy and take measures to control infection;furthermore,to attach great importance to continuous nursing care and improve the quality of home care. After above careful treatment,all the 6 children survived;they were discharged from hospital 3~4 months later after the operation of enterostomy restoration. Followed up 2~4 months,6 children recovered well.

    To summarize the perioperative nursing experience of a newborn with bacterial corneal ulcer undergoing corneal transplantation. Preoperative nursing points:to strengthen eye observation and protection to avoid corneal perforation;to properly administer systemic and eye medication to control infection. Postoperative nursing points:to closely observe the condition of corneal grafts to prevent postoperative complications;to provide eye drops on time after surgery to ensure medication safety;to emphasize pain management and provide nutritional support;to provide targeted health education to parents to ensure that the patient receives continuous care. Through multidisciplinary team collaboration,meticulous treatment and care,the corneal graft of the patient was clear,the suture was not loose,and there was no bleeding in the anterior chamber after 1 week of corneal transplantation. The vital signs were stable,and the patient was discharged from the hospital,and ophthalmic follow-up was conducted. After 7 weeks of surgery,the suture was successfully removed and the corneal transplantation was successful.

    To summarize the nursing experience of a patient with gastric cancer who developed cytokine release syndrome after using immune checkpoint inhibitors. Key points of nursing care:development of nursing assessment decisions with a holistic view to guide safe nursing care;taking into account the contradiction between bleeding and thrombosis and providing good care for upper gastrointestinal bleeding;implementing a nursing strategy focusing on cleaning and anti-infection for IV oral mucositis;implementing risk management for severe pulmonary lesions;providing good hormone medication care and discharge follow-up management. The patient was successfully discharged on 52nd day with a 3-month follow-up in good condition.

    Objective To systematically review and synthesize the psychological experience of kinesiophobia in patients with cardiac rehabilitation. Methods PubMed,Web of science,Journals@Ovid,Embase,CINAHL,PsycINFO,Cochrane Library,CNKI,SinoMed,WanFang Database,Vip Database,American Heart Association,European Society of Cardiology and American Association of Cardiovascular and Pulmonary Rehabilitation were searched to collect qualitative research on the psychological experience of cardiac rehabilitation patients with kinesiophobia. The retrieval time was from the establishment of the databases to Jun 2023. The literature was evaluated using the Australian JBI Quality Evaluation Criteria for Qualitative Research in Evidence-based Health Care Centres(2016),and the results were consolidated using an aggregative integration approach. Results A total of 45 results were extracted from 14 studies. Similar results were summarized into 10 groups,and 3 integrated results were synthesized as followed. Kinesiophobia was influenced by many factors;kinesiophobia affects the life experience of patients;strategies to reduce the level of kinesiophobia. Conclusion Nurses should pay more attention to psychological experience of kinesiophobia,and take the corresponding intervention measures to help patients overcome the psychological barriers of kinesiophobia,perfect personalized exercise programs,and improve the level of physical activity.

    Objective To evaluate the effectiveness of exercise for people with Parkinson’s disease on sleep quality. Methods Computerized retrieval of PubMed,Web of Science,Embase,the Cochrane Library,CINAHL,CNKI,WanFang Data,VIP,CBM was conducted to collect randomized controlled trials about the effect of exercise on people with Parkinson’s disease from inception to December,2022. There were 2 researchers who independently screened the literature,extracted the data and evaluated the risks of bias in the included studies. Meta-analysis was performed using RevMan 5.4 software. Results A total of 13 studies were included,with 874 patients. The result of meta-analysis show that the overall effect size of exercise intervention on sleep quality for people with Parkinson’s disease is significant(SMD=-0.54,95%CI=[-0.90,-0.19],P<0.01). Subgroup analysis show that the maximum effect size of intervention frequency is 4-5 times/week(SMD=-0.75);the maximum effect size of exercise intensity is light intensity(SMD=-2.19);the maximum effect size of a single intervention time is 40-55 minutes(SMD=-0.69);the maximum effect size of exercise type is traditional Chinese exercise(SMD=-0.63);the maximum effect size of intervention cycle is 12 weeks(SMD=-0.66). Conclusion Exercise intervention has significantly effects to improve sleep quality on Parkinson’s disease patients. It is a more effective way to improve sleep quality by exercising 4-5 times per week,while each exercise lasts about 40-55 min for 12 weeks in Traditional Chinese Medicine exercise with light intensity.

    Objective To design a storage device for lymph node specimens of gynecological malignant tumors and to evaluate its application effect. Methods The specimen depository is composed of 3 parts,including storage frame,partition plate and marking plate. 391 patients with gynecological malignant tumor surgeries requiring collection of retroperitoneal lymph nodes were selected in the operating room of a tertiary A hospital in Zhejiang Province from January to December 2022 in accordance with the operation schedule. They were randomly divided into an experimental group(n=196) and a control group(n=195). Patients in the experimental group used a newly developed storage device,while the control group was with traditional device. The incidence of additional verification specimens by doctors,the average time for disposing of lymph node specimens,and the job satisfaction survey of doctors and nurses were compared. Results Within a period of 12 months after utilizing this easily recognizable specimen storage device,the additional verification rate in the experimental group was 10.62% compared with the rate of 72.48% in the control group. The average time of disposing lymph node specimens was reduced from(8.3±0.9)to(5.2±0.3) minutes. Doctors and nurses in the experimental group reported a higher level of satisfaction for the specimen storage device (P<0.05). Conclusion Utilizing the newly designed specimen storage device can help medical staff to alleviate their workload,enhance their work efficiency,raise their job satisfaction,and ensure the accuracy of pathological results.

    Objective To design a multifunctional sputum suction device and verify its effectiveness. Methods A total of 50 patients using the multifunctional sputum suction device in a tertiary A hospital in Guangzhou from July to December 2022 were selected as the test group,from January to June 2022,while 49 patients using the traditional sputum suction device were taken as the control group. We compared the first suction preparation time,perfect rate of goods preparation,satisfaction of medical staff between the 2 groups. Results Compared with the control group,the time of preparation for the first sputum suction in the test group was shorter(t=-120.113,P<0.001),and the times of preparation in the test group were more than these in the control group(χ2=8.002,P=0.005). The satisfaction degree of nursing staff to the tidiness,practicability,convenience and timeliness of the sputum suction device was higher than that of the control group,and the difference was statistically significant(Z=-6.825,Z=-5.172,Z=-6.806,Z=-7.215,all P<0.001). Conclusion The multifunctional sputum suction device can reduce the time spent in preparing the items before suction,improve the rate of item preparation and the satisfaction of nursing staff.
