    Application of Interaction Model of Client Health Behavior in patients with coronary heart disease after stent implantation

    Objective To explore the effect of nursing interventions based on the Interaction Model of Client Health Behavior(IMCHB) in cardiac rehabilitation of patients with coronary heart disease after stent implantation. Methods The convenience sampling method was adopted to select 40 patients with coronary heart disease after stent implantation in the cardiovascular department of a tertiary A hospital in Wuhan from April to July 2022 as the experimental group,and this group received the cardiac rehabilitation guidance and nursing intervention based on the interactive model of health behavior. A total of 40 patients with coronary heart disease who were hospitalized from December 2021 to March 2022 after stent implantation were selected as the control group,and this group received the routine cardiac rehabilitation guidance and nursing. The 6-minute walking distance,low density lipoprotein levels,medication compliance scores,self-efficacy scores and quality of life scores before and after intervention were compared between the 2 groups. Results A total of 75 patients completed the study, with 38 in the experimental group and 37 in the control group. After the intervention,the 6 minutes walking distance of the experimental group was(526.79±40.70),which was higher than(469.41±55.82) in the control group. The level of low density lipoprotein was(1.49±0.39) in the experimental group,which was lower than (1.77±0.49) in the control group,and the differences were statistically significant(P<0.05). The score of medication compliance in experimental group was(7.29±0.35) points,which was higher than(6.33±0.37) points in the control group. The score of self-efficacy was(7.04±0.65),which was higher than(5.98±0.74) in the control group. The quality of life score was (64.47±6.25),which was higher than(55.43±5.94) in the control group,and the differences were statistically significant(P<0.001). Conclusion The cardiac rehabilitation guidance and nursing intervention based on the IMCHB can improve patients’ subjective initiative,so as to improve patients’ medication compliance,improve patients’ sense of self-efficacy and quality of life,promote patients’ cardiac rehabilitation.

    Effect of Internet-based Cognitive Behavior Therapy in women with abnormal fetal termination

    Objective To explore the effect of Internet-based Cognitive Behavior Therapy(ICBT) on anxiety,depression and sleep quality in women with abnormal fetal termination. Methods The convenience sampling method was adopted to select 95 pregnant women with abnormal fetal termination in a tertiary A-level women’s hospital in Hangzhou as the research subjects. They were randomly divided into an experimental group and a control group. The experimental group received the ICBT intervention,and the control group received the routine nursing intervention. The psychological status and sleep quality in 2 groups were compared and analyzed. Results 43 patients in the experimental group and 44 in the control group participated the study. After the intervention,the Generalized Anxiety Scale score was(4.97±1.82) points in the experimental group,and(7.09±3.54) points in the control group. The difference between 2 groups was statistically significant(t=-2.312,P=0.019). The Edinburgh Postpartum Depression Scale score was(6.54±3.22) points in the experimental group,and (9.07±5.29) points in the control group. The difference between 2 groups was statistically significant(t=-3.124,P=0.004). The Pittsburgh Sleep Quality Index score was (6.76±3.54) points in the experimental group,and (8.95±3.01) points in the control group. The difference between 2 groups was statistically significant(t=4.908,P=0.032). Conclusion The ICBT intervention can effectively alleviate maternal anxiety and depression of women with abnormal fetal termination,and improve sleep quality.

    Objective To explore the effect of preventive swallowing interventions in patients with partial laryngectomy. Methods Using convenience sampling method,75 patients who underwent partial laryngectomy for the first time in a tertiary hospital in Changchun from August 2020 to February 2022 were selected. They were divided into an experimental group(n=38) and a control group (n=37) according to the order of admission time. The control group was given routine care and basic swallowing training. The experimental group was given a combined intervention of Chin tuck against resistance and respiratory-swallowing coordination model on the basis of the control group,combined with in-hospital and out-of-hospital family rehabilitation for 1 month of preventive swallowing exercise. The differences in swallowing safety,swallowing-oral feeding function and quality of life between the 2 groups were compared at a day before enrollment,14 days after operation and 1 month after operation. Results The time,grouping and interaction effect of swallowing safety,swallowing-oral feeding and quality of life in the 2 groups were statistically significant(P<0.05). The indexes of the experimental group were better than those in the control group at 14 days and 1 month after partial laryngectomy(P<0.05). Conclusion The combined intervention model of preventive Chin tuck against resistance and respiratory-swallowing coordination exercise can enhance swallowing safety,swallowing function,restore functional oral feeding,facilitate quality of life,and promote early rehabilitation in patients with partial laryngectomy.

    Objective To develop multimodal somatosensory exercise system for patients after total thyroidectomy,and analyze its application effect. Methods Using the convenient sampling method,100 patients who underwent total thyroidectomy in the head and neck surgery department of a tertiary first-class hospital in Zhejiang Province from February 2021 to January 2022 were selected as the research subjects. They were divided into a test group and a control group according to the random number table method. In addition to routine home exercise,the test group received multimodal somatosensory exercise therapy on the basis of conventional nursing measures,and the control group received routine postoperative nursing measures. The fatigue,quality of life and exercise compliance of the 2 groups were compared at the fourth week,the eighth week and the twelfth week of intervention respectively. Results The results of repeated measurement ANOVA showed that the fatigue scores of the 2 groups decreased with the extension of the intervention time. The fatigue scores of the test group were lower than those of the control group at the fourth week,the eighth week and the twelfth week of the intervention,with a statistically significant difference(P<0.001). The overall quality of life scores of the test group were higher than those of the control group at the 8 th and 12 th week of the intervention(P<0.001). With the prolongation of the intervention time,the decline of the exercise completion rate and the rate of reaching the standard in the test group was smaller than that in the control group at different time periods,with a statistically significant difference(P<0.001). Conclusion Multimodal somatosensory exercise can improve the fatigue,quality of life and exercise compliance of patients with thyroid cancer after surgery. Patients should be encouraged to perform scientific and effective exercise after surgery.

    Objective To design a special protective shield for tracheostoma after total laryngostomy with the function of volume-controlled humidification,heating and filtration,and to evaluate its application effect in patients with laryngostomy after laryngeal cancer surgery. Methods The protective shield consists of 4 parts:a laryngostomy protection module fitting the neck curve,ventilation filter module,ear-hook continuous humidification module and humidification liquid constant temperature system. A total of 110 laryngeal cancer patients who underwent total laryngectomy in the otolaryngology department of a tertiary A hospital in Hangzhou from January 2020 to December 2021 were selected as the research subjects. The patients were divided into an experimental group and a control group by random number table,with 55 cases in each group. The experimental group was given airway care with the self-designed special protective shields for laryngostomy,while the control group was given routine airway care. The volume,color,and crusting degree of sputum,airway comfort,daily airway humidification,sleep quality,and whether return to society were compared between the 2 groups a month after discharge. Results The amount of phlegm in the airway of the experimental group was less than that of the control group;the degree of phlegm scab was less than that of the control group;the situation of social reintegration was better than that of the control group;the difference between the 2 groups was statistically significant(P<0.05). Conclusion The new type of laryngeal shields can effectively protect the patient’s airway in manual airway care,avoid foreign body invasion,reduce water loss of airway and sputum volume,prevent from sputum crusting,and it plays a role in continuing nursing after the patient is discharged conveniently and efficiently.

    Effect of different rehabilitation therapies on neck and shoulder function of patients after neck dissection:a network meta-analysis

    Objective To evaluate the effect of 8 types of rehabilitation therapies on neck and shoulder function of patients after neck dissection by network meta-analysis. Methods Randomized controlled trials(RCTs) on the effects of rehabilitation training on neck and shoulder function of patients after neck dissection were retrieved from Chinese and English databases. The retrieval time was from the establishment of the database to April 2022,and the retrieval was independently conducted by 2 researchers.Review Manager 5.3 was used for quality assessment,and ADDIS 1.16.8 software was used for network meta-analysis. Results 22 randomized controlled trials(RCTs) were included,with 1407 patients and 8 types of rehabilitation therapy.The results of network meta-analysis showed that in terms of Constant-Murley Score,the brief electrical stimulation(P=0.040),neck and shoulder aerobic exercise(P=0.020),step combined exercise(aerobic+resistance)(P=0.009),Tai Chi(P=0.040)and acupuncture(P=0.030) were better than conventional rehabilitation. In terms of Neck Dissection Impairment Index,only acupuncture(P=0.020) was better than conventional rehabilitation methods. In terms of Shoulder Pain and Disability Index,only stepwise combined exercise (aerobic + resistance)(P=0.030) was better than conventional rehabilitation methods. In terms of shoulder range of exercise(shoulder flexion,shoulder abduction),there was no significant difference between the pairwise comparisons among the exercise types(P>0.05).In terms of neck flexion to the affected side,stepwise combined exercise(aerobic exercise + resistance exercise)(P=0.004),neck and shoulder aerobic exercise(P=0.008),Tai Chi(P=0.006),and acupoint therapy(P=0.010) were better than conventional rehabilitation methods. In terms of flexion of the neck to the contralateral side,neck and shoulder aerobics (P=0.040) and acupoint therapy(P=0.030) were better than conventional rehabilitation methods. In terms of rotation of the neck to the contralateral side,neck and shoulder aerobics(P=0.020) was better than conventional rehabilitation methods. In the rotation of the neck to the affected side,neck and shoulder aerobics(P=0.030) was better than conventional rehabilitation methods. Conclusion Among the 8 rehabilitation therapies,acupuncture and moxibustion can mainly reduce shoulder pain and improve their ability to participate in social activities. Step combined exercise(aerobic + resistance) mainly has advantages in restoring shoulder joint dysfunction,and neck and shoulder aerobic exercise mainly has advantages in improving neck range of motion. However,due to the limitations of this study,further verification is still needed.

    Objective To retrieve and obtain the relevant evidence of swallowing function training for patients with head and neck cancer undergoing radiotherapy,and to summarize the best evidence for clinical references. Methods We systematically searched National Guideline Clearinghouse(NGC),Guidelines International Network(GIN),Scottish Intercollegiate Guidelines Network(SIGN),Joanna Briggs Institute(JBI),Cochrane Library,PubMed,UP To Date,Web of Science,Embase,CNKI,Wanfang Database,medlive for all evidence of swallowing function training in head and neck cancer patients undergoing radiotherapy,including clinical recommended practice,guidelines,expert consensuses,systematic reviews and randomized controlled trials.The retrieval period was from the inception of databases to August 2022. Results A total of 20 articles,including 1 clinical recommended practice,2 guidelines,5 expert consensuses,7 systematic reviews,and 5 randomized controlled trials. Through the evidence extraction and integration of the included literature,27 pieces of best evidence were finally summarized from the following 8 aspects,namely training purpose,training subject,training time,preparation before training,training scheme,radiotherapy suggestions,quality control and effect evaluation. Conclusion 27 pieces of best evidence were obtained based on the evidence summary method. In the process of nursing practice,nurses should select evidence in combination with specific clinical situations and patient needs,and relevant evidence should be updated in a timely manner.

    Establishment and practice of information system for prevention and treatment of hospital-acquired venous thromboembolism

    Objective To explore the application effect of the information system for prevention and treatment of hospital-acquired venous thromboembolism(HA-VTE) in HA-VTE management. Methods A team of HA-VTE management was set up and the information system for prevention and treatment of HA-VTE was established,which mainly included 4 content modules,namely assessment,early warning,decision-making,and monitoring,in order to achieve functions such as time points risk assessment,early warning assessment,prevention taboo assessment,auxiliary decision support,scoring supervision,and achieve coordinated prevention and control throughout the hospital. The system was officially clinically applied in October 2022. The hospital HA-VTE risk assessment rate,prevention rate among medium to high-risk patients,awareness rate of HA-VTE prevention knowledge among medium to high-risk patients,implementation rate of basic prevention measures among medium to high-risk patients,and incidence rate of HA-VTE were compared before(from April to September 2022) and after (from October 2022 to March 2023) the use of the system. Results After the application of the information system for prevention and treatment of HA-VTE,the risk assessment rate of HA-VTE has increased from 96.96% to 100%. The prevention rate of medium to high-risk patients increased from 37.93% to 46.86%(including the increase of drug prevention rate from 26.99% to 37.12%,and the increase of combined prevention rate from 2.16% to 4.66%). The awareness rate of HA-VTE prevention knowledge for medium to high-risk patients has increased from 87.76% to 96.84%,and the implementation rate of basic preventive measures has increased from 83.67% to 94.74%,with statistically significant differences(P<0.05). The incidence of HA-VTE has decreased from 0.066% to 0.046%. The difference was not statistically significant(P>0.05). Conclusion The information system for prevention and treatment of HA-VTE can enhance the compliance of nursing staff in risk assessment,provide accurate and effective decision-making basis for medical staff,and improve the current situation of HA-VTE management in hospitalized patients.

    The status and influencing factors of cancer-related cognitive impairment in patients with ovarian cancer

    Objective To investigate the current situation of cancer-related cognitive impairment(CRCI) in patients with ovarian cancer,analyze its influencing factors,and explore the correlation between subjective and objective assessment of cognitive impairment. Methods From May 2021 to February 2022,155 patients with ovarian cancer from Shanghai Cancer Center were investigated by general information questionnaire,the Montreal Cognitive Assessment Scale(MoCA) and the Perceived Deficit Questionnaire(PDQ). Results Ovarian cancer patients had a high incidence of CRCI. The average score of MoCA was (24.14±4.02),and the incidence of CRCI was 65.8%. The decrease in abstraction,delayed recall,and visuospatial/executive functions were the most signi-ficant. The average score of PDQ was 17.35±10.67,with retrospective memory and planning/organization dimensions being the most severe. Binary logistics regression analysis showed that education and recurrence were influencing factors of MoCA(P<0.05). Multiple linear regression analysis showed that education,living status and taking oral medication significantly affected the score of PDQ. The score of PDQ were significantly negatively correlated with the score of MoCA(r=-0.210,P=0.009). Conclusion The prevalence of CRCI is high in patients with ovarian cancer,mainly in the memory,abstraction,and visual space and executive function. It is recommended that healthcare workers could incorporate CRCI into the symptom management of patients with ovarian cancer,and implement evidence-based interventions to those with high-risk factors to improve their quality of life.

    Investigation and analysis of influencing factors related to financial burden among pulmonary artery hypertension patients

    Objective To investigate subjective and objective financial burden and its correlation with health-related quality of life(HRQOL) among pulmonary artery hypertension(PAH) patients. Methods From February to September 2022,we conducted convenience sampling method to select PAH patients who were followed up in a tertiary hospital,using General Information Questionnaire,Comprehensive Score for Financial Toxicity Functional Assessment of Chronic Illness Therapy(COST),and PAH-specific HRQOL measurement(EmpPHasis-10). Logistic regression model was used to analyze the independent influencing factors. Results 152 PAH patients were included,and 123(80.9%) patients experienced financial burden,and the mean score of subjective financial burden was 16.22±10.19. The multivariate linear regression analysis showed that pulmonary artery systolic pressure(PASP),6 minute walk distance(6MWD),social-economic status,catastrophic health care expenditures,and cost-related medication nonadherence(CRNA) were influencing factors of financial burden,which accounted 42.9% of the total variation. Financial burden was correlated with EmpPHasis-10(P<0.05). Conclusion The subjective and objective financial burden of PAH patients were common,and the HRQOL of patients with high financial burden level was seriously impaired. Medical staff should pay attention to and comprehensively screen the financial burden to prevent patients from becoming poor due to disease in the stage of disease management,so as to reduce the impact of financial burden on patients’ prognosis.

    Development of a knowledge-attitude-practice assessment questionnaire for the investigation of home bathing status of ostomy patients and the test of its reliability and validity

    Objective To develop a knowledge-attitude-practice assessment questionnaire for home bathing status of ostomy patients and to test its reliability and validity. Methods Based on the theory of knowledge,attitude and practice,the initial questionnaire was generated through literature review and group meetings. The final questionnaire was formulated by 2 rounds of Delphi expert consultations and pilot-testing. A convenience sample of 386 stoma patients who visited the stoma outpatient clinic of a tertiary hospital in Zhejiang Province from July to December 2022 were selected to examine the reliability and validity of the questionnaire. Results The final knowledge-attitude-practice assessment questionnaire included 3 dimensions of knowledge,attitude and behavior,with a total of 24 items. The CVI of each item in the questionnaire ranged from 0.800 to 1.000,with an average of 0.932. Exploratory factor analysis showed that cumulative contribution rate was 67.181%. Confirmatory factor analysis showed that the assessment questionnaire had a good fit:the chi-square degree of freedom ratio(χ2/df)was 1.510;the fitting goodness index(GFI) was 0.878;the comparative goodness of fit index(CFI)was 0.948;the root means square error of approximation(RMSEA) was 0.049. The total Cronbach’s α coefficient of the questionnaire was 0.922,and the Cronbach’s alpha coefficient of each dimension was 0.920,0.862,0.897;the test-retest reliability coefficient of each dimension was 0.856,0.942,0.926. Conclusion The knowledge-attitude-practice assessment questionnaire has good reliability and validity,and it can be used as a valid tool for medical staff to carry out assessment and education of home bathing for patients with stoma.

    Objective To extract,evaluate and summarize the relevant evidence of thirst management of patients with oral tracheal intubation in ICU,and to provide evidence bases for medical staff for scientific management. Methods The BMJ Best Practice,CINAHL,Cochrane Library,Clinical Trials,Embase,PubMed,UpToDate,Web of Science,Guidelines International Network(GIN),National Guideline Clearinghouse(NGC),National Institute for Health and Clinical Excellence(NICE),Joanna Briggs Institute(JBI),Registered Nurses’ Association of Ontario(RNAO),Scottish Intercollegiate Guidelines Network(SIGN),CNKI,SinoMed,Wan Fang database,VIP database and Yimaitong Guidelines Network were systematically searched for clinical decisions,evidence summaries,guidelines,recommended practices,expert consensuses,systematic reviews,and randomized controlled trials and the retrieval time limit is from the establishment of the database to November 20,2022. 2 researchers who were trained in evidence-based nursing independently evaluated the quality of the literature,and extracted and summarized the evidence in combination with expert suggestions. Results A total of 17 articles were included,including 4 clinical decisions,1 evidence summary,4 systematic reviews,and 8 randomized controlled trials. 26 pieces of best evidence were summarized on the management of thirst in ICU patients undergoing oral tracheal intubation,including 4 aspects:identification of risk factors for thirst,evaluation,intervention,and evaluation of the effectiveness of intervention measures. Conclusion The best evidence summarized in this study can provide evidence-based bases for clinical medical staff to manage the thirst symptoms of patients with oral tracheal intubation in ICU. When applying evidence,medical staff should correctly use the evidence in combination with the patient’s wishes and specific clinical situations to solve the patient’s thirst problem.

    Objective To retrieve and obtain the relevant evidence of standardized pulmonary rehabilitation strategies for patients with severe idiopathic scoliosis,and to summarize the best evidence. Methods We searched relevant guideline networks,PubMed,Embase,Web of Science,CNKI,Wanfang databases to collect relevant guidelines,expert consensuses,best practices,recommended practices and systematic evaluations related to pulmonary rehabilitation management in severe idiopathic scoliosis from the establishment of the database to January 30,2023. There were 2 researchers who independently evaluated the literature quality and extracted data. Results A total of 14 articles were included,including 2 best practices,3 guidelines,5 expert consensuses,4 systematic reviews,and 25 pieces of best evidence were summarized from 7 dimensions,namely clear objective of pulmonary rehabilitation,multidisciplinary team management,optimal timing of pulmonary rehabilitation,preoperative pulmonary rehabilitation strategy,intraoperative pulmonary protection strategy,postoperative pulmonary rehabilitation strategy,and compre-hensive intervention. Conclusion This study summarized the best evidence of pulmonary rehabilitation management for patients with severe idiopathic scoliosis,and it has a certain scientific and comprehensive nature,which can provide evidence-based bases for the formulating of pulmonary rehabilitation evaluation and management programs and practice standards for patients with severe idiopathic scoliosis.

    Objective To conduct a scoping review of studies related to the application of virtual roaming interventions in patient preoperative education,and to provide a reference for nursing educators to conduct similar studies or apply techniques in the future. Methods A search was conducted for studies published in Wanfang,CNKI,VIP,China Biomedical Abstracts Database,PubMed,Embase,Web of Science,CINAHL,Cochrane Library,from the date of database construction to September 30,2022. The literature in English and Chinese on the use of virtual roaming in patient preoperative education was collected,and the literature was screened according to inclusion and exclusion criteria. The information in the literature was extracted and summarized by 2 researchers independently. Results A total of 16 papers were included,and virtual roaming was used in patient preoperative education mainly in the form of panoramic videos. The main content included 3 parts,namely scenario reproduction,information support,and emotional management. The main evaluation elements included usefulness (mental state,knowledge level,behavior change,surgical experience) and ease of use. Conclusion Virtual roaming interventions can have good prospects for application in patient preoperative education,reducing negative emotions and improving preoperative readiness,which can achieve good health education outcomes.

    Telehealth intervention in the continuous nursing of patients with lung transplantation:a scoping review

    Objective To identify the specific interventions,content elements and application effects of telehealth,so as to provide the guidance for future research. Methods The literature was retrieved from PubMed,Embase,Cochrane Library,Web of Science,CNKI,Wanfang,and CBM databases. The included studies were summarized and analyzed. The search time was from the establishment of the databases to January 5th,2023. Results A total of 29 articles were included. The intervention forms included application,portable electronic device,network communication software,and website. The content elements included remote monitoring,education,rehabilitation,outpatient,psychological and cognitive intervention. The outcome types were mainly physiological indicators,psychological indicators,self-management,clinically outcome,and feasibility. Conclusion The main forms of continuous nursing are mobile applications and portable electronic devices. Comprehensive content and rich functions of devices are more favored by patients. Telehealth intervention is feasible,safe,and effective. Medical workers can further broaden the service of continuous nursing for patients with lung transplantation by remote technology. In the future,studies on telehealth intervention are still needed to promote the health of patients and improve the outcomes of patients.

    Objective To review the research status of Health Recommender System(HRS) in the field of cancer patient care. Methods Following the methodology of the scoping review,relevant literature was searched in Pubmed,Medline,Web of Science,Cochrane Library,CINAHL,Embase,China Biomedical Literature Database,CNKI,Wanfang and VIP Chinese journal databases. The search period was from the establishment until May 2022,and the included articles were summarized and analyzed. Results A total of 12 articles were included,and the application of HRS in the field of cancer patient care involved recommended diseases,treatment and health care related information,patient self-care guidance,symptom management,decision support,remote home management and follow-up. The evaluation indicators mainly include usability,effectiveness,accuracy and safety. Conclusion HRS can provide intelligent and personalized care support for cancer patients,but the research in the field of cancer is still in its infancy,and it is necessary to expand the application of HRS in the field of cancer patient care in the future,to integrate relevant theories to build HRS,to further improve the evaluation index,and to promote clinical application.

    Reliability and validity of Chinese version of the Work-Family Behavioral Role Conflict Scale

    Objective To translate the Work-family Behavioral Role Conflict Scale (WFBRCS) and test its reliability and validity. Methods The WFBRCS was translated and back translated according to the Brislin translation model,and the Chinese version of WFBRCS was formed after the cultural adjustment by the expert consultation and cognitive interview. 301 clinical nurses were investigated by convenience sampling method in a tertiary hospital in Shanghai to evaluate the reliability and validity of the scale from August to September 2022. Results A total of 268 nurses completed the survey. The Chinese version of WFBRCS included 2 dimensions with a total of 30 items. The content validity index of each item was 0.800~1.000,and the content validity index of the total scale was 0.980. Exploratory factor analysis resulted in a 2-factor structure,explaining 63.902% of the total variance. The Cronbach’s α coefficient of the total scale was 0.958,and the Cronbach’s α coefficients of the 2 dimensions both were 0.951. The retest reliability for the total scale was 0.849,and the retest reliability of the 2 dimensions were 0.856 and 0.813. Conclusion The Chinese version of WFBRCS has good reliability and validity,which can be used to evaluate the level of work-family behavior role conflict of nurses in Chinese cultural background.
