    Effects of occupational therapy on cognitive function in patients with Alzheimer’s disease

    Objective To construct the intervention effect of occupational therapy-based return to family intervention program on improving cognitive function and quality of life in patients with Alzheimer’s disease. Methods Through literature analysis and expert meeting,the return to family intervention program was formed. 68 patients with Alzheimer’s disease in the department of geriatric psychiatry who were admitted for the first time from April 2021 to October 2021 in a tertiary specialized hospital were selected. According to the community where the patients lived,Liangxi district was the study group and Huishan District was the control group,with 34 cases in each group. The study group carried out return to family intervention on the basis of routine nursing,including online communication,offline family visit,nursing clinic and community group activities,for 6 months. The control group received routine psychiatric nursing. The scores of Mini-Mental State Examination,modified Barthel Index Scale and Quality of Life Scale for Alzheimer’s patients were compared before the intervention,3 months and 6 months after the intervention. Results At 3 months and 6 months after the intervention,the scores of Mini-Mental State Examination,modified Barthel Index Scale,and the total score of Quality of Life Scale for patients with Alzheimer’s disease in the study group were higher than those before the intervention and the control group,and the differences were statistically significant(P<0.05). The compliances of the study group were 64.52% and 90.32% at 3 months and 6 months after intervention,respectively. Conclusion Return to family intervention based on occupational therapy can improve cognitive function,daily living ability and quality of life in patients with Alzheimer’s disease.

    Objective To build an intelligent interactive nursing information support system("AI Woman") and verify its application value in young breast cancer survivors. Methods The "AI Woman" system focuses on young breast cancer survivors and realizes multidimensional interaction based on artificial intelligence technology to provide personalized nursing information support. The convenience sampling was employed to select 100 young breast cancer survivors from 3 tertiary A hospitals in Shanghai from April 2021 to December 2021. They were randomly divided into 2 groups by systematic allocation. The control group(n=50) was given routine support in the information support system,and the intervention group(n=50) was given intelligent and interactive services. The symptom distress,self-efficacy,social support and quality of life of the 2 groups were compared before the intervention,1 month and 3 months after the intervention. Results After 1 month of intervention,the short-term and long-term indicators of the 2 groups showed a benign trend as a whole. The advantages of the intervention group were greater than those of the control group. After 3 months of intervention,45 cases in the intervention group and 42 cases in the control group completed the study. There were significant differences in symptom distress,self-efficacy and quality of life among groups,time and interaction(P<0.05). The scores of social support in the intervention group were higher than those in the control group(P<0.05). Conclusion The intelligent interactive nursing information support system constructed in this study is scientific and feasible,and plays a positive role in improving the physical and mental symptoms,self-efficacy and social support of young breast cancer survivors,and improving the quality of life during their survival.

    Fear of recurrence in cancer patients:a latent profile analysis

    Objective To explore potential classes characteristics of cancer patients’ fear of recurrence,and to analyze differences in characteristics of cancer patients across classes. Methods A cross-sectional study design with proportional allocation continuous sampling was used to select cancer patients of lung cancer,gastrointestinal cancer,breast cancer and gynecologic tumors who were hospitalized and treated in a tertiary hospital in Shenyang from February 2021 to October 2021 as the survey population. The study instruments included the General Information Questionnaire,the Illness Uncertainty Scale,the Hospital Anxiety and Depression Scale,the Perceived Social Support Scale and the short version of the Fear of Cancer Recurrence Scale. Results A total of 390 cancer patients were investigated,and 76.7% of them had fear of recurrence. Fear of recurrence was demonstrated in 3 different potential classes,named as “low fear of recurrence-extroverted”,“medium fear of recurrence-regular”,and “high fear of recurrence-neurotic”.The percentages were 19.5%,59.7%,and 20.8%,respectively.Compared with the type of “low fear of recurrence-extroverted”,cancer patients with higher anxiety levels were more likely to be classified as “medium fear of recurrence-regular” and “high fear of recurrence-neurotic”(OR=1.282,P=0.016;OR=1.378,P=0.010). Cancer patients with lower levels of social support were more likely to be classified as “medium fear of recurrence-regular” and “high fear of recurrence-neurotic” (OR=0.948,P=0.010;OR=0.900,P<0.001). Cancer patients with higher levels of illness uncertainty were more likely to be classified as “high fear of recurrence-neurotic”(OR=1.078,P=0.012). Conclusion The incidence of fear of recurrence in cancer patients is at a high level with distinct characteristics of classification. It is suggested that health care professionals can adopt targeted nursing interventions according to the personality characteristics of different classes of cancer patients to avoid fear of recurrence.

    Objective This study aimed to describe social isolation-related psychological experiences of cancer patients,in order to provide practicable references for developing relevant nursing support programs. Methods This descriptive qualitative study,involving a semi-structured in-depth interview of 20 cancer patients based on purposive sampling,was conducted from November to December 2021 at a tertiary A tumor hospital in Shanghai. The qualitative content analysis was performed in order to summarize and extract themes from the interview texts. Results The cancer isolation-related psychological experiences of cancer patients were extracted and classified into 3 themes,which should be dealt in a positive and comprehensive way in order to produce a supportive environment for survivors:social withdrawal(distress of cancer-related symptoms,worries regarding health impairment,stigma regarding change of body image and social prejudice,lack of interest in social activities);hiding of inner feelings (the psychological distress tolerated by oneself,worries about influencing others,failure to communicate with others);eagerness for support(reliance on family support,feelings of gratefulness for the support of friends,eagerness for seeking professional support). Conclusion The study found that cancer patients suffered social isolation because of their impaired physical health,poor psychological status and insufficient social support. Medical staff should strengthen the symptom management and psychological counseling provided to these survivors and enhance their family and social support,in order to help cancer survivors return and adapt to society.

    Development and evaluation of a risk predictive model for high level of self-reported symptom cluster distress in breast cancer patients undergoing chemotherapy

    Objective To develop and evaluate a risk predictive model for high level of self-reported symptom cluster distress in breast cancer patients undergoing chemotherapy. Methods 647 patients who received chemotherapy and met the inclusion and exclusion criteria were selected at 4 tertiary hospitals in Shanghai,Shandong,Jiangsu,and Zhejiang from October 2019 to May 2021. Cases were randomly assigned to a modeling group and a validation group based on 5-fold cross-validation method at a ratio of 8:2. The random forest algorithm was used to develop the risk predictive model in the modeling group. The receiver operating characteristic curve,Hosmer-Lemeshow goodness-of-fit test,calibration curve,and decision curve were used to comprehensively evaluate the prediction performance of the model in the validation group. Risk factors were identified based on the order of the importance of each influencing factors. Results The incidence of high symptom cluster distress was 33.27% in the modeling group and 29.23% in the validation group. The area under the receiver operating characteristic curve of the prediction model was 0.91;the sensitivity was 65.8%;the specificity was 93.5%;Hosmer-Lemeshow goodness-of-fit test was insignificant(P=0.1365);the decision curve was above the reference line. Body image,self-efficacy,and financial burden were the most important predictors. Conclusion The risk predictive model based on random forest algorithm has good predictive performance,which is of great significance to help identify breast cancer subgroups at high risk of symptom cluster distress,and will potentially promote symptom management.

    Objective To construct and validate a risk predictive model for treatment delay of patients with lung cancer. Methods A convenience sampling method was used to select 493 lung cancer patients admitted to a tertiary care hospital in Guangxi from September 2021 to June 2022. They were randomly divided into 345 cases in a modeling group and 148 cases in an internal validation group,and 47 lung cancer patients admitted to another tertiary care hospital were selected as the external validation group. The risk prediction model and nomograms model were constructed using logistic regression analysis and R software,and the area under the receiver operating characteristic(ROC) curve was used to test the prediction effect of the model. Results The established model was Y=ez/(1+ez),Z=9.04 × low level of disease perception + 2.01 × irregular physical examination-0.08 × perceived social support scale score + 0.23 × score of perceived barriers to healthcare-seeking decision-Chinese -0.15 × score of Health Literacy Scale for Chronic Patients. The ROC of the internal validation group was 0.849;the sensitivity was 82.39%;the specificity was 78.83%;the positive predictive was 90.61%;the negative predictive was 63.57%. The ROC of the external validation group was 0.830;the sensitivity was 97.36%;the specificity was 80.03%;the positive predictive was 94.62%;the negative predictive was 80.05%. Conclusion The low level of disease perception,lack of regular physical examination,the low level of social support,the high impaired perception of medical seeking behavior,and the low level of health literacy are the influencing factors of treatment delay of lung cancer patients,and the prediction model based on the above factors has a good degree of differentiation and calibration,which can help to identify the high-risk treatment delay lung cancer individuals. Nurses can identify high-risk groups based on this model,so as to take measures to improve their medical behavior and reduce the risk of treatment delay.

    For cancer patients of childbearing age,the reproductive and sexual dysfunction caused by treatment will significantly affect their mental health and quality of life. Therefore,it is particularly important to make therapeutic decisions for fertility preservation at an early stage. Patient decision aids can help patients understand fertility treatment options and instruct the decision process,which are useful supplements for fertility preservation information. In this review,the significance of fertility preservation for cancer patients,types,application approaches,effects and problems of decision aids are summarized,aiming to provide references for the development of decision aids for fertility preservation for cancer patients in China.

    Objective To translate the Quality of Working Life Questionnaire for Cancer Survivors(QWLQ-CS) into Chinese,and to test its reliability and validity. Methods According to the Brislin translation-back translation model,the English version of the QWLQ-CS was translated,back-translated and cultural adjusted,and 250 cancer survivors who returned to work were selected by convenient sampling method to investigate its reliability and validity in a tertiary A hospital in Zhengzhou from April 2021 to April 2022. Results The QWLQ-CS in Chinese version includes 5 dimensions with a total of 23 items. The I-CVI was 0.833-1.000;the S-CVI/UA was 0.826;the S-CVI/Ave was 0.971;the structure validity χ2/df was 2.312;the approximate error mean square root (RM-SEA) was 0.074;the comparative fit index(CFI) was 0.918 and Tucker-Lewis coefficient(TLI) was 0.905. The internal consistency index Cronbach’s α coefficient was 0.895,and the retest reliability was 0.864. Conclusion The QWLQ-CS in Chinese version is of good reliability and validity,which is suitable for the assessment of the life quality of cancer survivors returning to work in the context of Chinese culture.

    Study on the application of a phased oral feeding promotion program in preterm infants with gestational age less than 32 weeks

    Objective To investigate the effect of a phased oral feeding promotion program in preterm infants with gestational age less than 32 weeks. Methods 128 preterm infants less than 32 gestational weeks admitted to the neonatal intensive care unit(NICU) of a tertiary Children’s Hospital in Shanghai from January to December 2020 were consecutively enrolled as the subjects of this study. They were randomly divided into an experimental group and a control group by block method. The modified phased oral feeding promotion program was applied in the experimental group. The control group received the traditional feeding method. The days of life,corrected gestational age and weight of 2 groups at attaining full oral feeding,at the beginning of oral feeding and the first oral intake of 50% of the meal were compared. The hospital stays and the occurrence of feeding related complications were also compared. Results There were 5 cases lost,and finally 61 cases were included in the experimental group and 62 cases were included in the control group. The corrected gestational age at attaining full oral feeding in the experimental group was lower than that in the control group(P<0.05). The days of life,corrected gestational age and body weight of the experimental group were significantly lower than those of the control group at the beginning of oral feeding and the first oral intake of 50% of this meal(P<0.05). There was no significant difference between 2 groups in hospital stay(P>0.05). No feeding related complications occurred in 2 groups. Conclusion For premature infants with gestational age less than 32 weeks,the phased oral feeding promotion program can achieve full oral feeding at an earlier time.

    The effects of health literacy on decision satisfaction of decision-makers in patients undergoing cardiac macrovascular surgery

    Objective To explore the mediating effects of perceived stress and uncertainty tolerance on health literacy and decision satisfaction of decision makers in patient undergoing cardiac macrovascular surgery,and to provide references for improving the satisfaction of decision-making. Methods 260 surgical patient’s decision-makers in the cardiac macrovascular surgery of a hospital in Wuhan were selected by convenience sampling. All Aspects of Health Literacy Scale,Perceived Stress Scale,Intolerance of Uncertainty Scale-12 and decision satisfaction scale were used to investigate. Results 251 valid questionnaires were recovered,with an effective recovery rate of 96.54%. The health literacy score was(23.05±5.61);the perceived stress score was(20.31±6.08);the uncertainty tolerance score was(26.41±5.94);the decision satisfaction score was(43.86±10.54). The 4 variables were correlated(P<0.001). The mediating model found that health literacy had a significant direct effect on decision satisfaction. Meanwhile,health literacy also indirectly affected decision satisfaction through 3 paths,including the independent mediating effect of perceived stress(β=0.096,P<0.001),the independent mediating effect of uncertainty tolerance (β=0.093,P=0.001),the chain mediating effect of perceived stress and uncertainty tolerance(β=0.076,P<0.001),accounting for 49.17% of the total effect. Conclusion Perceived stress and uncertainty tolerance play a mediating role in the relationship between health literacy and decision satisfaction of decision makers in cardiac macro-vascular surgery,which can improve decision makers’ health literacy,reduce perceived stress,and help them perceive surgical risks rationally,thus improving decision satisfaction.

    Study on sedentary behavior and its influencing factors in patients with arteriosclerosis obliterans

    Objective To investigate the current status of sedentary behavior and its influencing factors in patients with atherosclerosis obliterans(ASO). Methods This study conveniently selected 148 patients with ASO who were hospitalized in the Department of Vascular Surgery of a tertiary hospital in Nanchang from October 2020 to January 2022 as the survey subjects. They were investigated using the general information questionnaire,the Chinese adult sedentary behavior questionnaire,the Barthel index,perceived social support scale(PSSS),short physical performance battery(SPPB) and visual analogue scale(VAS). Results The daily sedentary time of ASO patients is 9.13±0.99 hours. Barthel index,PSSS,SPPB,and VAS scores were 82.19±16.58,43.18±6.70,8.23±2.01 and 4.26±1.03. The results of multiple linear regression analysis showed that age,number of comorbid chronic diseases,activities of daily living,perceived social support and lower extremity limb function were the main influencing factors of sedentary behavior in ASO patients. Conclusion Patients with ASO have high levels of sedentary behavior,and health professionals could develop targeted interventions to reduce the occurrence of sedentary behavior in patients by combining the appropriate influencing factors.

    Investigation on the development trajectories of exercise fear in patients with chronic obstructive pulmonary disease

    Objective To explore the development trajectories of exercise fear and its influencing factors in COPD patients. Methods From January 2022 to July 2022,216 patients with COPD who were hospitalized in the respiratory department of a tertiary A hospital in Huzhou City were selected as subjects by convenience sampling method. The Tampa Scale of Kinesiophobia was used to evaluate their exercise fear level at the time of discharge,and 1 month,3 months and 6 months after discharge. The latent class growth model was used to classify the exercise fear trajectory,and the influencing factors of different types of exercise fear were analyzed. Results The trajectory of exercise fear in COPD patients was identified into 3 potential categories:low exercise fear group (25.5%),moderate exercise fear group(46.8%) and high exercise fear group(27.7%). There were significant differences in age,course of disease,BMI,lung function,educational level,pain,fatigue and mMRC among different trajectory types(P<0.05). Conclusion Within 6 months after discharge of COPD patients,most patients’ exercise fear has been effectively alleviated,and there is group heterogeneity in their exercise fear. Nurses should carry out targeted nursing measures according to the patients’ exercise fear trajectory and its influencing factors,in order to improve and reduce the patients’ exercise fear state.

    Objective To systematically evaluate the caring experience of family caregivers of aged patients with multi-morbidity, and to provide a reference for developing individualized support strategies for family caregivers of multi-morbidity patients. Methods Qualitative studies on the caring experience of family caregivers were searched in electronic databases, including Web of Science, PubMed, Embase, the Cochrane Library, CINAHL, CNKI, WanFang, VIP database, and CBM Database. The retrieval time was from the establishment of the databases to August 2022. The literature was evaluated using the Australian JBI Quality Evaluation Criteria for Qualitative Research in Evidence-based Health Care Centres(2016), and the results were consolidated using an aggregative integration approach. Results A total of 10 studies were included, and 54 clear research results were extracted. The similar research results were summarized into 11 new categories, and 3 integrated results were synthesized. The caring experience has multiple effects on the caregiver’s mind, body and life;caregivers face multiple challenges;caregivers respond positively to maintain a normal life. Conclusion Medical staff should attach importance to the caring experience of family caregivers of elderly patients with multi-morbidity, promote and construct a caregiver support security system, develop personalized support strategies, meet the needs of caregivers, and improve their quality of care and quality of life.

    Objective To extract,summarize and evaluate the best evidence of self-management for patients with knee osteoarthritis(KOA),and to provide evidence-based evidence for improving self-management of patients with KOA. Methods We searched the guidelines websites,association official websites and integrated databases,for all literature on self-management of patients with KOA;the types of literature included clinical decision-making,guidelines,evidence summaries,expert-consensuses,systematic reviews and randomized controlled trials. The retrieval time was from January 2012 to October 2022. Results A total of 25 articles were included,including 11 guidelines,4 expert consensuses,1 clinical decision,4 systematic reviews and 5 randomized controlled trials. A total of 30 pieces of evidence were extracted. After sorting and discussion,the evidence was integrated into 4 areas,namely learning KOA related knowledge,exercise management,weight management and joint protection. Conclusion The evidence of self-management of patients with KOA is abundant,high quality and authoritative,which can provide evidence-based bases for clinical medical staff,so as to help patients with KOA carry out self-management more scientifically and effectively.

    Prevalence of sleep disorders in post-ICU patients: a systematic review

    Objective To systematically evaluate the prevalence of sleep disorders and provide evidence for early prevention of sleep disorders in post-ICU patients. Methods PubMed,Embase,Web of Science,the Cochrane Library,CINAHL,CBM,CNKI,WanFang Data and VIP were searched from inception to June 2022. There were 2 researchers who independently screened the literature and assessed the quality of included studies using Critical Appraisal Tools published by Newcastle-Ottawa Quality Scale(NOS). The meta-analysis was performed by StataSE.12 software. Results A total of 28 studies were included in this study with a total of 3 170 patients. The prevalence of subjective sleep disorders by random effects model was 51.84%[95%CI(45.17%~58.51%)]. The results of subgroup analysis showed that the prevalence of subjective sleep disorders within 1 month,1~3 months,>3~6 months and >6 months after ICU discharge was 50.51%[95%CI(40.97%~60.05%)]、53.34%[95%CI(44.86%~61.82%)]、49.44%[95%CI(32.71%~66.16%)]、29.55%[95%CI(13.28%~45.82%)],respectively. Meta regression results of single covariate showed that sample size and country were related to heterogeneity(P<0.05). Conclusion The prevalence of sleep disorders in post-ICU patients is high. Nursing staff should take relevant interventions,strengthen sleep health education and guidance for patients,and reduce the incidence.

    The symptom science model integrates 4 core elements (complex symptom,phenotype characterization,biobehavioral factors,and clinical application) and 3 influencing factors(patient-centered experience,social determinants of health,policy and population health) to explore the symptom assessment,biological mechanisms and management of chronic diseases. The symptom science model emphasizes the important role of genomics,proteomics,microbiomics,and metabolomics and other methods in the study of symptom science,promotes nurses’ understanding of the potential biological mechanisms and biomarkers of symptoms,and develops targeted,cost-effective and innovative interventions and control measures to prevent,manage or improve the symptoms of chronic diseases. This study describes the formation and framework of symptom science model,analyses the application status of symptom science model in chronic disease nursing care,and provides references for multidisciplinary integration development in chronic disease symptom management.

    Clinical practice guidelines are important bridges between research-based evidence and clinical practice. Therefore,adherence to clinical practice guidelines by nurses will improve the scientific nature of the nursing practice. However,the implementation process of clinical practice guidelines is complex,which results in low adherence of recommended guidelines,and thus affecting the scientificity of nursing practice. The development,dissemination,and implementation of clinical guidelines is a continuous process. Several factors may affect guideline adherence in various levels. This paper summarizes the developmental history,status,evaluation methods,and factors influencing guideline adherence based on nursing fields. Possible countermeasures and suggestions are proposed for improving guideline adherence,and provide theoretical references for the formulation,dissemination,and application of clinical guidelines.

    Objective To sinicize Tyndall-Bailey Falls Risk Assessment Tool For ICU (ICU-FRAT) and test its reliability and validity. Methods Based on Brislin model,the original scale was translated,back-translated,checked and translated,cultural adjusted and pretest to form the Chinese version of ICU-FRAT. A total of 200 ICU patients from 3 hospitals in Nanjing from March to May 2022 were investigated by convenience sampling to evaluate the reliability and validity of the scale. Results The Chinese version of ICU-FRAT has a total of 10 items,with item level content validity index of 0.83 to 1.00,overall consistency level content validity index of 0.90,and average level content validity index of 0.98. After exploratory factor analysis,3 common factors were extracted,and the cumulative variance contribution rate was 79.32%. Confirmatory factor analysis suggested that each test quantity of scale model fit was within an acceptable range. The Cronbach’s α coefficient of the total scale was 0.857;the Cronbach’s α coefficient of each item ranged from 0.830 to 0.867;the half reliability was 0.838. Conclusion The Chinese version of the ICU-FRAT has good reliability and validity,and it can be used to assess the risk of falls in ICU patients.
