    Development and application of an early nourishing enteral nutrition process in patients with severe stroke

    Objective To develop a process for early tonic enteral nutrition in patients with severe stroke and to evaluate its effectiveness. Methods We developed an early nourishing enteral nutrition process for patients with severe stroke through literature search,best evidence summaries,expert consultations and clinical trials. A convenience sampling method was used to consecutively select patients with severe stroke admitted to the neurology ward of a tertiary care hospital in Suzhou from January 2020 to December 2021 as the study population. Among them,patients admitted from January 2021 to December 2021 were set as the test group with a total of 49 cases,and those from January 2020 to December 2020 were set as the control group with a total of 50 cases. The test group implemented the early nourishing enteral nutrition process,and the control group implemented the conventional enteral nutrition support treatment,comparing the occurrence of feeding intolerance and nutritional indicators between the 2 groups. Results The incidences of diarrhea,constipation and feeding interruption in the test group were lower than those in the control group significantly(P<0.05). The hemoglobin value in the control group decreased significantly after 7 days of hospitalization,compared with that at the time of admission(P<0.05),but the hemoglobin value in the test group remained basically balanced with that at the time of admission after 7 days of hospitalization,and the difference was not statistically significant(P>0.05). The plasma albumin level in both the test and control groups decreased significantly after 7 days of hospitalization,compared with that at the time of admission(P<0.05),but the decrease in the test group was lower than that in the control group significantly(P=0.046). Conclusion Early nourishing enteral nutrition process can reduce the incidence of diarrhea,constipation and feeding interruptions significantly,while promoting the recovery of hemoglobin and slowing the decline of albumin levels in patients with severe stroke.

    Nutritional risk prediction model for elderly hospitalized patients with mental disorders

    Objective To construct a nutritional risk prediction model for elderly inpatients with mental disorders,and to provide a basis for clinical nutritional status assessment. Methods Using the method of cluster sampling,584 elderly patients with mental disorders who were hospitalized in Beijing Huilongguan Hospital from June 2020 to June 2021 were divided into a good nutritional status group(152 cases) and a malnutrition risk group(129 cases) and a malnutrition group(303 cases). The general condition questionnaire,frailty screening scale (FP),mini-nutrition assessment scale(MNA),simplified physical performance scale(SPPB),the Barthel Index(BI) of daily life and Family Care Index(APGAR) for ability evaluation were used for investigation. Logistic regression analysis and Graphpad Prism software were used to draw receiver operating characteristic curve to test the model fit and prediction effect. Results The incidence of malnutrition in elderly hospitalized patients with mental disorders was 51.88%,and advanced age(OR=1.049),frailty(OR=2.578),and BMI(OR=0.629) were independent risk factors for malnutrition risks(P<0.534). 0.05),in which advanced age was the greatest risk factor for malnutrition. Advanced age(OR=1.031),diagnosis(OR=2.150),frailty(OR=2.485),and physical dysfunction(OR=2.123) were independent risks of malnutrition(P<0.05),among which frailty is the greatest risk factor for malnutrition risk to be transformed into malnutrition. The area under the receiver operating characteristic curve(AUC) of the malnutrition risk is 0.732;the Youden index is 0.388;the optimal critical value is 0.473;the sensitivity of the prediction model is 0.736;the specificity is 0.651. Regarding the ROC curve of malnutrition,AUC is 0.808;Youden index is 0.531;the best critical value is 0.581;the sensitivity of the prediction model is 0.865;the specificity is 0.667. Those indicate that the 2 prediction models are accurate and reliable. Conclusion The risk prediction model of nutritional status of hospitalized elderly patients with mental disorders constructed in this study has high accuracy,and provides a basis for risk assessment and prevention of nutritional status of elderly patients with mental disorders in hospital.

    Objective To explore the risk factors of enteral nutrition-associated diarrhea(ENAD),and to construct a risk prediction model based on random forest. Methods This study adopted a retrospective data collection method. A total of 537 ICU patients who received enteral nutrition in a tertiary hospital in Tianjin from August 2017 to May 2021 were enrolled. Based on univariate analysis and multivariate logistic regression analysis and VIF,the variables of the risk prediction model for enteral nutrition-associated diarrhea were determined. All data are randomly divided into a training set and a validation set by 6:4. Based on the training set data,the random forest model of ENAD is established,and the importance of ENAD risk factors was ranked by variable importance score using a random forest model. Confusion matrix analysis,accuracy,sensitivity,specificity,and area under the ROC curve were used to evaluate the predictive effect of the model on the validation dataset. Results The incidence of ENAD in this study was 38.73%. After univariate analysis,multivariate Logistic regression analysis and VIF,9 variables were determined to be included in the prediction model. The top 5 variable importance scores of the random forest prediction model include days of taking potassium preparations,APACHE Ⅱ score,daily doses of enteral nutrition,fasting days before enteral nutrition and hypoproteinemia. The accuracy of the random forest model on the validation set is 76.27%;the sensitivity is 70.73%;the specificity is 79.70%;the area under the ROC curve is 0.810(95%CI:0.638~0.827). Conclusion The predictive model has good predictive performance,which provides a reference for the precise prevention of clinical nurses with certain practical significance.

    Objective To establish an enteral nutrition risk assessment and management scheme for patients with decompensated liver cirrhosis and to provide a reference for clinical practice. Methods The literature related to the risk assessment and management of enteral nutrition in patients with decompensated liver cirrhosis was searched in domestic and international databases. The research team members formed an enteral nutrition risk and management scheme primarily for patients with decompensated liver cirrhosis,and then 2 rounds of expert correspondence were conducted to revise the draft and determine the content by Delphi method from September to November 2021. Results 14 experts were included,and the authoritative coefficients of the 2 rounds of correspondence with experts were 0.838 and 0.839,respectively. The Kendal harmony coefficient and expert coordination coefficient of the primary,secondary,and tertiary indicators were 0.836,0.714 and 0.683 in the second-round correspondence,and the coefficient of variation of each indicator ranged from 0 to 0.23. A final version of nutritional risk assessment and management scheme for patients with decompensated liver cirrhosis was finally constructed,including 5 first-level indicators,14 second-level indicators,and 18 third-level indicators. Conclusion The established enteral nutrition risk assessment and management scheme for patients with decompensated liver cirrhosis is rigorous,scientific,and reliable,which can provide a reference for clinical practice.

    Objective To evaluate and summarize the evidence of nutritional support for pressure injuries in elderly inpatients,so as to provide references for clinical nursing care. Methods BMJ Best Practices,UpToDate,RNAO,NGC,GIN,NICE,SIGN,JBI,Cochrane Library,PubMed,EMBASE,CNKI,Wanfang database were searched by computer,as well as the EPUAP,WOCN and ESPEN websites,in order to collect the relevant literature on nutritional support for pressure injuries in elderly inpatients. The retrieval period was from the establishment of the database to June 2022. The JBI evidence appraisal and recommendation system were used to evaluate the quality and evidence level of the included literature. Results A total of 7 articles were included,including 6 guidelines and 1 evidence summary. 19 pieces of evidence were summarized from 5 aspects,including nutrition risk screening,comprehensive nutrition assessment,individualized nutrition care plan,nutrition intervention and health education. Conclusion This study summarizes the best evidence of nutritional support for elderly inpatients with pressure injuries,and clinical medical staff can follow the best evidence to develop individualized nutrition care plans for elderly inpatients with pressure injuries.

    Objective To search and select the evidence related to perioperative nutritional management of gastric cancer,and to summarize the best evidence. Methods We retrieved the BMJ best practice,Up to Date,Guidelines International Network,Medlive,National Institute for Health and Care Excellence,Scottish Intercollegiate Guidelines Network,Registered Nurses Association of Ontario,JBI,National Comprehensive Cancer Network,European Society for Clinical Nutrition and Metabolism,Chinese Society For Parenteral and Enteral Nutrition,American Society for Parenteral and Enteral Nutrition,Cochrane Library,PubMed,CNKI,Wanfang Database,VIP,SinoMed,and collected relevant clinical decisions,guidelines,evidence summaries,expert consensuses and systematic reviews. The search time limit was from January,2012 to April,2022. There were 2 researchers who evaluated the quality of the included literature,and extracted and summarized the evidence. Results A total of 15 articles were included,including 1 clinical decision,4 guidelines,1 evidence summary,5 expert consensuses,and 4 systematic reviews. 28 pieces of evidence were summarized,including nutritional risk screening and assessment,multidisciplinary team,general principles of nutritional support,preoperative nutritional support,postoperative nutritional support,discharge and follow-up. Conclusion The best evidence in this study is scientific and systematic,providing evidence-based interventions for medical workers to implement nutritional management of perioperative gastric cancer patients.

    Objective To evaluate the effect of physical restraint reduction strategies on the improvement of restraint practice of Intensive Care Unit(ICU) patients with invasive mechanical ventilation. Methods Patients with invasive mechanical ventilation admitted to ICU of our hospital from February 2021 to February 2022 were enrolled and divided into a control group and a study group according to random number table method. The control group adopted the conventional physical restraint method,and the study group adopted the physical restraint reduction strategy. The duration of physical restraint,unplanned extubation rate,delirium incidence,duration of invasive mechanical ventilation and days in ICU were compared between the 2 groups. Results 113 patients with 78 males and 35 females were eventually enrolled in the study,including 57 patients in the study group and 56 patients in the control group. There were no significant differences in physical restraint rate,unplanned extubation rate and skin abnormality rate between the study group and the control group(P>0.05). The duration of physical restraint,delirium rate,duration of invasive mechanical ventilation and days in ICU in the control group were higher than those in the study group. The cumulative use rate of physical restraint was significantly different between the 2 groups(Log Rank test χ2=18.491,P<0.001). The cumulative utilization rate of invasive mechanical ventilation was significantly different between the 2 groups(Log Rank test χ2=9.244,P=0.002). Conclusion The physical restraint reduction strategy could reduce the duration of physical restraint,the incidence of delirium,the duration of invasive mechanical ventilation and the length of stay in ICU.

    Analysis of potential categories of psychological resilience in primary brain tumor patients

    Objective To explore the potential categories and influencing factors of psychological resilience in primary brain tumor patients. Methods A convenient sampling method was used to select 237 primary brain tumor patients hospitalized in the Neurosurgery Department of a tertiary hospital in Kunming,Yunnan Province from December 2020 to August 2021. General information questionnaire,Resilience Scale-14(RS-14),Medical Coping Modes Questionnaire(MCMQ) and Perceived Social Support Scale(PSSS) were conducted for investigation. Latent class analysis (LCA) was used to explore the potential categories of psychological resilience among patients with primary brain tumor. Single factor analysis,analysis of variance and ordinal and multinomial logistic regression analysis were used to identify the factors associated with the potential categories of psychological resilience. Results A total of 226 returned questionnaires were valid,resulting an effective response rate of 95.4%. Based on the analysis of potential categories,the psychological resilience of patients with primary brain tumor was divided into 3 potential categories,namely low level group (27.4%),medium-high fluctuation group(30.1%) and high level group(42.5%). The result of ordinal and multinomial logistic regression analysis showed that the score of PSSS(OR=1.045,P<0.001),facing dimension(OR=1.102,P=0.016) and yield dimension(OR=0.865,P=0.013) in MCMQ,and rural residence (OR=0.437,P=0.049) were the influencing factors of the potential categories of psychological resilience. Conclusion Patients with primary brain tumor have obvious differences in characteristics by psychological resilience. More awareness and interventions from healthcare providers based on characteristics of different types of patients and influencing factors are needed,in order to promote the psychological resilience.

    Objective To develop a suicide-related mental health service behavioral tendencies questionnaire and to test its reliability and validity. Methods Based on the theory of planned behavior,on the basis of literature analysis and semi-structured interviews,the initial questionnaire was formed through expert correspondence and pre-surveys. From May 2020 to July 2020,542 medical staff were selected for a survey to test the reliability and validity of the questionnaire. Results The questionnaire included 4 dimensions and 26 items;exploratory factor analysis extracted 4 common factors,and the cumulative variance contribution rate was 65.525%;confirmatory factor analysis showed that the fitting index of the questionnaire structure model was good. The content validity index of the questionnaire was 0.924. The Cronbach’s alpha coefficient of the total questionnaire was 0.942,and the split-half reliability was 0.910,and the test-retest reliability was 0.905. Conclusion The suicide-related mental health service behavioral tendencies questionnaire has good reliability and validity,and it can be used as a behavioral tendencies tool to evaluate the mental health services provided by medical staff to suicide patients.

    Effect of Sizi powder wax therapy on Three Yin Muscle Meridians of arm for upper limb motor dysfunction in stroke patients with hemiplegia

    Objective To observe the effects of Sizi powder wax therapy on the area of Three Yin Muscle Meridians of arm for upper limb motor function,spasticity and quality of life for upper limb motor dysfunction in stroke patients with hemiplegia. Methods The stroke patients with upper limb hemiplegia who were admitted to the Affiliated Rehabilitation Hospital of Fujian University of Traditional Chinese Medicine from May 2020 to January 2021 were selected. 141 patients were randomly divided into a Sizi powder wax therapy group,a wax therapy group and a control group with 47 cases in each group. All patients in the 3 groups received the routine treatment,nursing and rehabilitation training protocol. On this basis,the Sizi powder wax therapy group received Sizi powder wax therapy on the Three Yin Muscle Meridian of arm;the wax therapy group received wax therapy on the Three Yin Muscle Meridian of arm,once a day,20 minutes each time,5 times per week for 4 weeks. The upper limb motor function and degree of spasticity were evaluated before the intervention,2 weeks after the intervention and 4 weeks after the intervention,and the quality of life was evaluated before the intervention and after the intervention. Results A total of 130 cases completed the trial. In the Sizi powder wax therapy group,1 case was excluded,3 cases fell off and 1 case was discontinued. In the wax therapy group,1 case was excluded,2 cases fell off and 1 case was discontinued. In the control group,1 case was excluded and 1 case fell off. After 4 weeks of intervention,the improvement of Upper Limb Fugl-Meyer Assessment Scale(U-FMA) and Modified Ashworth Scale(MAS) in Sizi powder wax therapy group was higher than that in control group(P<0.05);the Stroke Specific Quality of Life(SS-QOL) scores of Sizi powder wax therapy group and wax therapy group were better than those of the control group(P<0.05). The results of Generalized Estimation Equation showed that the time effect and interaction effect of U-FMA score were statistically significant(P<0.05),and the time effect of clinical efficiency of spasticity was statistically significant(P<0.05). Conclusion Sizi powder wax therapy on the Three Yin Muscle Meridian of arm can improve upper limb motor function,spasticity and quality of life of patients with upper limb motor dysfunction after stroke.

    The status quo and influencing factors of post competency among nurses in central sterile service departments

    Objective To investigate the level of post competency among nurses in central sterile service departments(CSSD) and to analyze its influencing factors,so as to provide references for improving the quality construction for nursing teams in CSSD. Methods A total of 3 312 nurses in CSSD of hospitals in 31 provinces and autonomous regions were selected from October 2021 by the convenient sampling method. The nurses were investigated by the demographic information questionnaire and Post Competency Assessment Scale for nurses in CSSD. Multiple linear regression analysis was used to study the influencing factors of nurses’ post competency. Results The total score of post competency of nurses in CSSD was(4.32±0.55). Multiple linear regression analysis showed that gender,years of working in CSSD,hospital grade,number of operations per month,centralized management,energy level management and post performance management were the influencing factors of post competency of nurses in CSSD. Conclusion CSSD nurses generally mark high scores on the post competency assessment. Male CSSD nurses,longer working years,higher tier of hospitals,and monthly operations would tend to have higher post competency of nurses. For hospitals executing centralized sterile supply management,competence level management and post performance management,their nurses tend to score higher on post competency assessment as well. Managers should pay attention to the evaluation and intervention of post competency of nurses in CSSD,and formulate targeted post competency training plans based on related factors to improve the post competency of nurses in CSSD and promote the sustainable development of this special profession.

    Construction of an exercise-combined cognitive intervention program among Chinese community-dwelling older adults with reversible cognitive frailty

    Objective To construct an exercise combined cognitive intervention program for the older adults with reversible cognitive frailty in the community. Methods The first draft of the program was formulated by literature analysis with the reference to guidelines and expert consensuses related to exercise and cognitive intervention. Results 10 experts were included. The recovery rates of the 2 rounds of questionnaires were 80% and 100%,respectively. The expert authority coefficients of the exercise intervention program and cognitive intervention program were 0.90 and 0.99,respectively. The mean importance score of each item was 4.25~5.00,and the full score ratio was 0.25~1.00. The coordination coefficients of expert opinions were 0.429(P=0.002) and 0.545(P=0.031). The coefficient of variation of each item was 0~0.133. The results were credible and desirable. Finally,the exercise-combined cognitive intervention program for older adults with reversible cognitive frailty was formed. Conclusion The exercise combined cognitive intervention program for older adults with reversible cognitive frailty in community is scientific and reliable,and it can be used to improve physical and cognitive functions of older adults,to delay the occurrence and development of adverse outcomes.

    To summarize the lung rehabilitation nursing experience of a case of severe diffuse axonal injury with pulmonary contusion. The key points of nursing care include establishing a multidisciplinary lung rehabilitation management team,carrying out personalized respiratory support,using descending step sequential oxygen therapy,giving precise clearance of the respiratory tract,and carrying out early strategies of lung rehabilitation based on the integrated mode of “health,care,rehabilitation,and patient”. With the effort of multidisciplinary team cooperation,the ventilator was removed 12 days after admission. The patient received extubation on the 17th day after admission and got out of bed on the 20th day after admission,and was discharged smoothly on the 37th day after admission. During the 6-month follow-up,the patient recovered well.

    To summarize the nursing experience of a patient with acute ST-segment elevation myocardial infarction complicated with epilepsy. The main nursing care are as follows:initiating a multidisciplinary response team to formulate the best diagnosis,treatment and nursing plan;identifying epilepsy seizure and emergency nursing care;strengthening disease observation and nursing to prevent north-south syndrome;conducting individualized and progressive rehabilitation training to prevent ICU acquired weakness.     Objective To systematically evaluate the qualitative research on the experience and feeling of elderly patients with hip fracture during rehabilitation. Methods We searched databases including the Cochrane Library,PubMed,Embase,CBM,CNKI,Wanfang,VIP. The literature of qualitative research on the experience and needs of rehabilitation for elderly patients with hip fracture from the establishment of the database to December 2021 was retrieved. The literature was evaluated using the Australian JBI Quality Evaluation Criteria for Qualitative Research in Evidence-based Health Care Centres(2016),and the results were consolidated using an aggregative integration approach. Results A total of 29 studies were included;3 theoretical models and 93 clear research topics were extracted,and 12 new categories were formed after induction and integration,which were finally merged into 4 integrated results. Integration results 1:Limitations of trauma on physical status of elderly patients with hip fracture. Integration results 2:The effect of trauma on the psychological state of elderly patients with hip fracture. Integration results 3:The elderly patients with hip fracture showed different attitudes and coping styles during rehabilitation. Integrated results 4:The needs of elderly patients with hip fracture during rehabilitation. Conclusion The elderly patients with hip fracture have dual physical and mental limitations after surgery,and will experience a long and struggling process. Medical staff should pay attention to patients diversified and multi-dimensional rehabilitation needs and formulate individualized rehabilitation strategies based on patients’ needs.

    Objective To evaluate and summarize the relevant evidence of falling prevention in patients with Parkinson’s disease,in order to provide references for clinical medical staff. Methods The evidence-based nursing method was used to systematically retrieve the clinical decisions,guidelines,evidence summaries,systematic evaluations and expert consensuses on the prevention of falls in patients with Parkinson’s disease in relevant databases at home and abroad and professional academic websites of Parkinson’s disease. The retrieval time limit was from the establishment of the databases to August 31,2021. After the methodological quality evaluation,the evidence was extracted and summarized according to the subject. Results A total of 18 pieces of the literature were involved,including 2 clinical decisions,2 guidelines,1 evidence summary,12 systematic reviews and 1 expert consensus. Combined with the judgment of professionals,the evidence was summarized from 5 aspects:evaluation and monitoring,daily safety management,sports management,nutritional support and education and training,and 24 pieces of best evidence were formed. Conclusion This study comprehensively and scientifically summarizes the best evidence of fall prevention and management plan for patients with Parkinson’s disease. In the process of evidence transformation,evidence users should consider specific situations,pertinently select the evidence,formulate the individualized fall prevention and management plan for patients with Parkinson’s disease,reduce the risk of falls and improve the quality of care.

    Pre-exposure prophylaxis refers to a biological prevention method to reduce the probability of HIV infection by taking drugs when people are at high risk of HIV infection. Medication compliance is the key cause of determining the effectiveness of pre-exposure prophylaxis. This article discusses the medication compliance assessment tools of pre-exposure prophylaxis developed at home and abroad which includes the main contents of assessment tools,application status and advantages and disadvantages,expecting to provide references for accurately evaluating medication compliance of high-risk population of AIDS and formulating intervention programs.
