    Effect of external diaphragm pacer on prevention of ventilator-induced diaphragmatic dysfunction in patients with ICU non-invasive ventilation

    Objective To establish the rehabilitation program of external diaphragm pacer and to explore its preventive effect on ventilator-induced diaphragmatic dysfunction(VIDD) in patients with ICU non-invasive mechanical ventilation. Methods Through the literature review,group meeting and pre-experiment,a program was formulated to prevent VIDD in patients with non-invasive ventilation in ICU. Patients with non-invasive ventilation were conveniently selected from September 2020 to June 2021 in a respiratory intensive care unit of a tertiary hospital in Henan province. They were divided into 2 groups by random number table method. On the basis of routine nursing care,the experimental group was given the external diaphragm pacer training program,and the control group was given routine nursing program. The incidence of VIDD,diaphragm thickness score,diaphragm displacement and blood gas analysis were compared between 2 groups after the intervention. Results A total of 74 patients were included with 10 cases fallen off,and finally 64 cases were included with 32 cases in the experimental group and 32 cases in the control group. After the intervention,the incidence of VIDD was 6.25% in the experimental group,while that was 43.75% in the control group. The difference between 2 groups was statistically significant(P=0.001). Conclusion The external diaphragm pacer rehabilitation program can reduce the incidence of VIDD and improve diaphragm and respiratory function in patients with ICU non-invasive mechanical ventilation.

    Longitudinal study of kinesiophobia trajectory in patients with acute myocardial infarction after percuta-neous coronary intervention

    Objective To explore the status and development trend of kinesiophobia after percutaneous coronary intervention(PCI) in patients with acute myocardial infarction(AMI),and to provide evidence for the best intervention time point of kinesiophobia. Methods From November 2020 to June 2021,a total of 105 patients with AMI after PCI admitted to the Department of Cardiology of tertiary A general hospitals in Hangzhou were selected by convenience sampling method. The general data questionnaire,tampa scale of kinesiophobia-heart and cardiac anxiety questionnaire were used to investigate and observe the degree of kinetophobia and psychogenic anxiety at 3 time points(within 1 week of hospitalization,2 weeks after discharge and 3 months after discharge). Results The kinesiophobia scores of AMI patients at 3 time points after PCI were(40.82±5.40),(41.90±4.60),(37.40±3.35),which were higher than the norm(32.20±7.40). The results of repeated measure ANOVA showed that different time,different age groups and cardiac anxiety significantly affected the score of kinesiophobia(P=0.022,P=0.048,P<0.001). After the control for covariates,different time had a significant interaction with age(P=0.013),T1 and T2 time had significant interaction with cardiac anxiety(P=0.001,P<0.001). Conclusion Due to the interaction of time, age and cardiac anxiety, the level of kinetophobia in AMI patients was higher after PCI and within 3 months after discharge. At T2 time point, the score of kinesiophobia and cardiac anxiety in the young and middle-age groups was significantly higher than those in the elderly group. The trajectory of kinesiophobia showed a slight increase in acute convalescence period,and a natural trend of gradual decrease in middle and post convalescence period. The kineophobia kinesiophobia of AMI patients after PCI lasts for a long time with a wide impact,so it is necessary to timely carry out interventions.

    Objective To investigate the effects of intervention strategies based on self-efficacy theory on nutritional status and cancer-induced fatigue(CRF) after total thoracoscopic esophageal cancer. Methods A total of 119 patients with esophageal cancer admitted to the Second Affiliated Hospital of Nanchang University from January 2017 to December 2020 were selected by convenience sampling method. They were divided into an experimental group(n=59) and a control group(n=60) according to random number method. Patients in the control group received conventional nursing,while patients in the experiment group take the self-efficacy theory as the instruction intervention strategy. The self-efficacy,nutritional status and T4 CRF of 2 groups of patients were compared in the hospital(T0),1 month after discharge(T1),3 months after discharge(T2),6 months after discharge (T3),and 1 year after discharge(T4). Results The self-efficacy score of the experimental group was higher than that of the control group at different time points after intervention,and the risk score of nutritional status was lower than that of the control group,with statistical significances(P<0.05). At T4,the CRF score of the experiment group (33.90±6.30) was significantly lower than that of control group(44.93±7.66)(P<0.001). Conclusion Nursing intervention strategies based on self-efficacy theory can significantly increase the self-efficacy of patients after thoracoscopic esophageal cancer surgery,improve their nutritional status,and alleviate CRF.

    Objective To explore the influencing factors of the transition to adults on children with cancer from the perspective of professional care providers,so as to improve their transition readiness. Methods From September 2020 to June 2021,by phenomenological research methods,semi-structured in-depth interviews were conducted with 5 doctors,19 nurses and 3 social workers. The interview data were transcribed,coded,analyzed and refined. Results From the perspective of professional care providers,the factors influencing the transition of children with cancer can be summarized into 4 factors,i.e. individual,family,medical system and society,and 9 sub-factors,i.e. diagnosis and treatment experience,disease cognition,autonomy,transitional consciousness,education style,professional team,profes-sional service,social environment and service system. Conclusion There are multiple factors influencing the transition to adulthood. To assist them to effectively cope with the long-term impact of disease and treatment,it is necessary to raise the public attention,encourage children to be informed and participate in disease management,upgrade professionals’ capabilities,strengthen the continuity of long-term follow-up,develop the service system,and develop individualized and targeted intervention adapted to Chinese situations.

    Construction of sexual health care training program for cervical cancer patients

    Objective To construct a training program of sexual health care for patients with cervical cancer for gynecological oncology nurses,and provide references for the theoretical research and clinical practice of improving the sexual health care ability of gynecological oncology nurses for patients with cervical cancer. Methods Based on literature review and qualitative interview,the first draft of a letter inquiring about the training program of sexual health care on cervical cancer patients for gynecological oncology nurses was formulated. From September to December 2020,15 gynecological oncology medical experts,gynecological oncology nursing experts and sexual science experts from 4 provinces and cities of Beijing,Tianjin,Shanghai and Jilin Province were interviewed by Delphi experts for 2 rounds to formulate the training program of sexual health care on cervical cancer patients for gynecological oncology nurses. Results The effective recovery rates of 2 rounds of expert consultations were 93.75% and 100%. The authority coefficients of the authority coefficients were 0.843 and 0.853.The Kendall’ W was 0.429 and 0.326. The coefficient of variation of items in the second round was 0~0.115,and the full score rate is 20%~100%.The constructed training program of sexual health care on cervical cancer patients for gynecological oncology nurses consists of 3 parts,among which the training content includes 6 first-level indicators,16 second-level indicators,48 third-level indicators,and 6 training methods and 6 assessment methods. Conclusion The process of constructing training program of sexual health care on cervical cancer patients for gynecological oncology nurses is authentic and reliable,with distinctive specialty features,various training methods and assessment methods,which can meet the needs of sexual health care training for gynecological oncology nurses,and can promote the scientific and standardized development of sexual health care.

    Objective To select and summary the best evidence on prevention and management of the skin adverse reactions in patients with targeted tumor therapy,and to provide references for clinical decision. Methods We systematically searched for the evidence on prevention and management of the skin adverse reactions in patients with targeted tumor therapy in BMJ best practice,UpToDate clinical decision system,Joanna Briggs Institute,Guidelines International Network,National Institute for Health and Clinical Excellence,Scottish Intercollegiate Guidelines Network,Yi Maitong Guidelines Network,National Guideline Clearinghouse,the Agency for Healthcare Research and Quality,Registered Nurses’ Association of Ontario,World Oncology Network,National Comprehensive Cancer Network,Lung Cancer Committee of China Anti-Cancer Association,Chinese Anti-Cancer Association Oncologic Nursing Comittee,Cochrane Library,Embase,CHINAL,Web of Science,OVID,PubMed,SinoMed,CNKI,WanFang database,VIP database to collect the literature,including clinical guidelines,evidence summaries,best practice information sheets,recommended practice,systematic reviews and original research. The retrieval period was from January 1,2011 to October 3,2021.There were 2 researchers evaluating the quality of the literature and extracting the data. Results 15 articles were selected,including 3 guidelines,4 evidence summaries,1 systematic reviews,3 randomized controlled trials,1 class experiment and 3 expert consensuses. 19 pieces of best evidence were formed from 6 aspects,including skin evaluation,risk factors,drug therapy,skin care,skin adverse reaction management and health education. Conclusion This study summarizes the best evidence on management and prevention of skin adverse reactions in patients with targeted tumor therapy,and provides evidence-based evidence for clinical workers and managers. It is suggested that clinical staff should combine clinical context,fully assess patient’s skin condition,do a good job of skin health education,examination and care to prevent the occurrence of skin adverse reactions.

    Effectiveness of acceptance and commitment therapy on breast cancer patients:a Meta-analysis

    Objective To systematically evaluate the intervention effect of acceptance and commitment therapy on breast cancer patients. Methods A systematic search of databases, including the Cochrane Library, Web of Science, PubMed, Embase, CINAHL, PsycInfo, CNKI, WanFang Data and CBM, was carried out to collect randomized controlled trials on acceptance and commitment therapy on breast cancer patients from inception to August, 2021. There were 2 researchers who screened the literature according to inclusion and exclusion criteria, extracted data and evaluated the methodological quality. Meta-analysis was conducted with Revman5.4 software. Results A total of 13 randomized controlled studies were included, involving 1061 patients. Meta-analysis showed that acceptance and commitment therapy was superior to conventional care on improving depression [SMD=-1.15, 95%CI(-1.48, -0.82), P<0.001], anxiety [SMD=-1.28, 95%CI(-1.53, -1.04), P<0.001], psychological flexibility [MD=-12.26, 95%CI(-15.02, -9.51), P<0.001] and post-traumatic growth [MD=-12.48, 95%CI(-13.55, -11.40), P<0.001], and perceived benefit of disease[MD=-8.77, 95%CI(-10.79, -6.75), P<0.001] of breast cancer patients. Conclusion Acceptance and commitment therapy can relieve depression and anxiety, improve psychological flexibility and post-traumatic growth, and enhance the perceived benefit of disease. It plays a positive role in the improvement of mental state of breast cancer patients.

    A scoping review of models for predicting the risk of unplanned readmission in cancer patients

    Objective A scoping review of unplanned readmission(UR) risk prediction models for cancer patients was conducted to provide a basis for clinical practice and research. Methods The UR risk prediction model of cancer patients was focused,and the Chinese and English databases were searched systematically. The extracted information of the model included applicable population,the incidence of UR,modeling methodology,predictors of the model and their performance. Results 18 studies involving 23 prediction models were included and the population focused on postoperative colorectal cancer patients. The incidence of 30 days UR in cancer patients ranged from 8.2% to 19.0%. The model development methods were various,but the overall prediction performance was poor. Comorbidities,TNM,length of stay,age and postoperative complications were important predictors of UR in cancer patients. Conclusion Clinical staff should pay attention to UR risk factors and choose excellent tools to guide clinical practice. Prediction models with high predictive performance and operability can be developed with artificial intelligence and verified extensively and externally.

    Objective To clarify the concept of the social isolation with cancer patients. Methods We searched studies on social isolation with cancer patients from websites and databases including Wanfang,CNKI,CBM,VIP,Pubmed,Web of Science,EBSCO,Embase,Proquest,Springer Link,Elsevier Science Direct,Wiley Online Library,Scopus. We selected papers according to inclusion criteria. Rodgers evolutionary method of concept analysis was used. Results A total of 103 articles were retrieved,and 5 attributes were identified,including the lack of contacts,self-isolation,loneliness,alienation and meaninglessness. Antecedents included demographic factors,factors of disease and treatment factors,psychological and sociological factors. Consequences included the increased bad behavior and decreased physical functioning. Conclusion The concept attributes of social isolation of cancer patients were identified by concept analysis method. In the future,researchers can carry out more comprehensive and in-depth research,combining the attributes of social isolation with cancer patients.

    Currently,the survival time of colorectal cancer patients has been gradually extended,but cancer and treatment can cause many troubles for them. Patient-reported outcomes with colorectal cancer focus on assessing the physical and psychological experience of patients,which helps to identify their health challenges and give targeted treatment. This article reviews commonly used patient-reported outcome measures in the field of colorectal cancer at home and abroad and their clinical application. The content characteristics,existing problems of the current tools were comparatively analyzed. Future research directions are proposed. It is intended to provide a reference for the application,development,and clinical practice of patient-reported outcome measures for colorectal cancer patients in China.

    Objective To investigate the cognition,attitude and practice of pediatric ICU nurses on daily sedation interruption(DSI),and to provide references for clinical training. Methods Convenience sampling was used to conduct a questionnaire survey among 298 pediatric ICU nurses in 7 tertiary A children hospitals in China in July 2021. The general information questionnaire,questionnaires of knowledge,attitude,and behavior on DSI were self-designed. Results In terms of the cognition of 298 pediatric nurses on DSI,their knowledge score was 4(2,4) out of 10,and a total of 157 nurses(52.68%) said they did not know well or had never heard of DSI. In terms of attitude,their recognition score was 32(27,34) out of 40,and 217 nurses(72.82%) were willing to implement DSI,and 258 nurses(86.58%) wanted systematic training. In practice of nurses,221(74.16%) have not received relevant training,and 159 nurses(53.35%) never or rarely performed DSI. Conclusion Pediatric ICU nurses have a poor grasp of knowledge on DSI,while their recognition toward DSI is high. Their practice needs to be strengthened. Nursing managers should pay full attention to the training needs of pediatric ICU nurses and carry out systematic training on DSI basic knowledge and practical skills,so as to improve DSI practice level of nurses.

    Analysis on the prevalence and risk factors of skin injuries in the elderly in 50 hospitals in China

    Objective To analyze the prevalence and risk factors of 3 types of skin injuries in the elderly through multicenter cross-sectional survey,so as to provide bases for formulating prevention strategies for skin injuries in the elderly. Methods On March 31,2021,1 067 nurses from 50 hospitals in 16 provinces,2 autonomous regions and 2 municipalities included inpatients over 60 years old and inspected their skin from head to toes to identify pressure injury(PI),incontinence-associated dermatitis(IAD) and skin tear(ST),and meanwhile collected demographic,healthcare condition and PI risk assessment data. Statistical software was used for descriptive analysis,Pearson correlation analysis and logistic regression analysis. Results Totally,14 470 cases were obtained. The average age was(73.35±8.90) years old. The total prevalence of skin injuries was 6.69%,and the prevalence of PI,IAD and ST was 3.91%,1.89% and 0.89%,respectively. Logistic regression analysis showed that the risk of PI,inability of self-care,hypoproteinemia and anemia,comorbidity over 2 chronic diseases and ≥70 years old were the independent risk factors of PI in the elderly. The risk of PI,inability of self-care,hypoproteinemia,comorbidity over 2 chronic diseases,length of hospital stay≥8 days and the age of over 80 years were the independent risk factors of IAD in the elderly. The risk of PI and hypoproteinemia were independent risk factors of ST in the elderly. Conclusion Skin injuries of the hospitalized elderly are the result of multiple factors,while PI and IAD are the most typical ones,and the risk factors of different types of skin injuries are also different. When formulating prevention strategies,we should not only pay attention to the independent role of risk factors,but also consider the combined role of risk factors,take comprehensive preventive measures to improve the prevention effect.

    Objective To explore the effect of cognition-early warning intervention on the cognitive behavior of patients with delayed treatment of diabetes in rural areas,in order to improve their treatment compliance. Methods Rural patients with delayed treatment of diabetes in Datong District,Daqing City,in Heilongjiang Province and Luannan County,Tangshan City,in Hebei Province were conveniently selected as the research subjects from October 2019 to January 2020. According to the random number table method,6 villages were divided into an experimental group and a control group,with 63 cases in each group. The experimental group was given the cognition-early warning intervention for 2 weeks,and the control group was given routine health education. The general demographic data of patients with delayed treatment of diabetes in rural areas were obtained through a general data questionnaire;the cognitive behavior scale of delayed treatment of diabetes in rural areas,medication compliance questionnaire and diabetes management self-efficacy scale were used to evaluate the cognitive behavior,medication compliance and self-efficacy scores of subjects before and after intervention;fasting glucose results were collected by venous blood collection. Results A total of 60 samples were included in the experimental group and the control group respectively. Before intervention,general demographic data,cognitive scores,medication compliance scores and diabetes management self-efficacy scores were comparable between the 2 groups. After intervention,the cognitive behavioral scores and medication compliance scores of the experimental group were higher than these of the control group and these before intervention,and the difference was statistically significant(P<0.05). The scores of diabetes management self-efficacy in experimental group were higher than these of the control group,with statistical significance(P<0.05). The self-efficacy score of diabetes management in experimental group was slightly higher than before intervention. Fasting blood glucose level in the experimental group was better than in the control group after intervention,with statistical significance(P<0.05). Conclusion Cognition-early warning intervention can improve the cognitive and behavioral level of patients with delayed treatment of diabetes in rural areas,and contribute to the standardized treatment of patients with delayed treatment of diabetes in rural areas.

    Development of a self-rating scale of nurses’ humanistic literacy and its reliability and validity test

    Objective To explore the constituent elements of nurses’ humanistic literacy,to construct the self-rating scale of nurses’ humanistic literacy and verify its reliability and validity. Methods From July to December 2020,on the basis of reviewing the latest literature,the initial item pool of the scale was formed through 2 rounds of expert consultation and pre-investigation. This scale was tested among 1 000 nurses in hospitals across the country. SPSS was used to analyze the reliability,and AMOS was used for confirmatory factor analysis. Results The self-rating scale of nurses’ humanistic literacy consisted of 6 dimensions and 54 items. The Cronbach’s α coefficient of the total scale was 0.982;the split-half reliability of the total scale was 0.962;the retest reliability of the total scale was 0.758. 6 common factors were extracted by exploratory factor analysis,which explained 68.089% of the total variance. The one-dimension/multi-factor model’s /df was 2.495;NFI was 0.800;modified standardized moderation index was 0.876;CFI was 0.922;TLI was 0.915,and RMSEA was 0.055. Conclusion This study determined the constituent elements of humanistic literacy of nurses in China. The scale with good reliability and validity can further provide support for the investigation of the humanistic literacy level of nurses in China. It can also provide a basis and reference for nursing management to develop interventions.

Nursing essentials include:nursing care of high flow vein - extracorporeal membrane oxygenation therapy with continuous renal replacement therapy of pressure monitoring;pipeline safety nursing of patients with the waking extracorporeal membrane oxygenation treatment,nursing care of continuous delirium condition;early recognition and treatment of blood coagulation dysfunction in the digestive tract hemorrhage,deep vein thrombosis prevention and nursing care;infection prevention and control of immunosuppressive state,nursing care of early lung rehabilitation and psychological intervention of patients. The patient was hospitalized for 51 days. After careful treatment and nursing,the patient gets better and was discharged.

    Objective To retrieve,evaluate and summarize the evidence related to blood collection via central venous access devices for clinical references. Methods Systematic search was conducted in related websites and databases. The search period was from the establishment of the databases to December 7,2021,and the clinical decisions,systematic reviews,guidelines,evidence summaries,and expert consensuses were included. The quality of the literature was independently evaluated by evidence-based trained investigators and combined with professional judgment to extract information from the literature that met the quality standards. Results A total of 7 articles were included,including 6 guidelines and 1 systematic review. The best evidence included a total of 27 pieces of evidence in 4 areas,namely training of health care personnel,blood collection methods,collection of special samples,and quality control strategies for blood collection. Conclusion In the process of evidence transformation,medical staff should comprehensively consider the clinical contexts and individual factors of patients,and choose the best evidence,optimize the clinical behavior of blood specimen collection through central venous access devices,so as to improve the standardization and safety of blood specimen collection.

    In terms of stroke prevention and treatment,the most important work are early monitoring of stroke risk factors,efforts to reduce the incidence of stroke in the population,and integration and identification of stroke predictors. Wearable devices and machine learning based on stroke risk factors are expected to become an effective solution for risk monitoring and prediction for people at high risk of stroke. This article reviews the monitoring methods for high-risk factors of stroke based on wearable devices,the application of machine learning and mobile phone applications in stroke risk prediction,which provides references for improving stroke prevention and health management in China in the future.

    Objective To translate and revise Hand Hygiene Behaviour Questionnaire(HHBQ) based on COM-B (capability,opportunity,motivation-behaviour) model,and to test its reliability and validity. Methods According to the intercultural debugging guidelines,the Chinese version of HHBQ was prepared by pre-translation,pre-translation synthesis,back-translation,back-translation synthesis,original author review,expert consultation and pre-test. The reliability and validity of Chinese version of HHBQ were evaluated by a survey of 537 medical staff from 3 tertiary A hospitals in Hangzhou from July to October 2021. Results The Chinese version of HHBQ contained 3 dimensions,namely,hand hygiene ability(5 items),hand hygiene opportunity(7 items),and hand hygiene motivation (5 items). The content validity of the total scale was 0.995,and the content validity of each item in the questionnaire was 0.923~1.000. 3 common factors were extracted from exploratory factor analysis,and the cumulative variance contribution rate was 68.741%. Confirmatory factor analysis showed that the model fit well. The Cronbach’s α coefficients of the total amount table are 0.918,and the Cronbach’s α coefficients of each dimension are 0.871~0.906. The retest reliability of the questionnaire was 0.739. Conclusion The Chinese version of HHBQ has good reliability and validity,and it can be used to comprehensively evaluate the influencing factors of medical staff’s hand hygiene.
