    Formulation and application of new nurse training model in ICU

    Objective To explore the application effect of the new ICU training program based on the role transition shock model. Methods A total of 100 ICU new nurses were enrolled in July 2018 as research subjects and were randomly allocated into the experimental and the control group. The experimental group was trained based on the training program formulated by the role transition shock model,while the control group was given regular training. The training effect was evaluated by the Transition Shock of Newly Graduated Nurses Scale(TSNGNS) and the 6-Dimension Scale of Nursing Performance(6-D). Results After training,the total score of the ICU TSNGNS and the score of each dimension in the experimental group were lower than those in the control group with a statistically significant difference(P<0.05). For the 6-D,the total score and the score of each dimension of the ICU new nurses in the experimental group were higher than those in the control group,and the difference was statistically significant(P<0.05). Conclusion The training program can alleviate the transition shock of the new nurses in ICU and improve their competency.

    Objective To develop an evaluation index system of continuous care for stroke patients,so as to provide a comprehensive,systematic and quantifiable reference for standardizing the evaluation of continuous care for stroke patients. Methods Based on literature search,semi-structured interview and group discussion,the first draft of index system was established and consultation questionnaire was compiled. Two rounds of consultation were conducted with 17 Chinese experts and Analytic Hierarchy Process was adopted to determine the weights of indicators of all levels. Results The response rates of two rounds of expert consultation were 100.0% and 94.1% respectively;authority coefficients were 0.930 and 0.935,respectively;coefficients of concordance were 0.451 and 0.409 respectively(P<0.01). The final evaluation index system of continuous care for stroke patients consisted of 4 first-level indicators(information continuity,management continuity,relationship continuity,and outcome of patients),15 second-level indicators and 56 third-level indicators. Conclusion The evaluation index system of continuous care for stroke patients are scientific,reliable and comprehensive in contents,which can provide science-based references for standardizing evaluation of continuous care for stroke patients.

    Objective To construct a nursing quality evaluation index system for ICU patient experience,and to provide references for objectively evaluating the quality of experience in ICU patients. Methods Under the guidance of evidence-based nursing practice methods,a pool of patient experience quality indicators was established by integrating results of literature inquiry and semi-structured interviews. The original questionnaire was determined by expert meeting,and the indicators were determined after 2 rounds of Delphi expert consultation. Experts’ opinions were summarized;9 indicators were deleted;6 indicators were revised; 5 indicators were added. The indicators were evaluated according to the criteria of the indicators,reliability of the experts and opinions of the research group. Results This study constructed 84 nursing quality indicators of ICU patient experience,including 5 first-level indicators,19 second-level indicators,and 60 third-level indicators;the average score of all indicators was >4 points,and the coefficient of variation was<0.25,indicating that the indicators had high reliability. Conclusion The nursing quality index system of ICU patient experience constructed in this study is highly scientific and effective,with guiding significance for evaluating the quality of experience in ICU patients.

    Objective To investigate the current practice of specialist nurses in China. Methods A self-designed questionnaire was used to conduct a survey among 53,316 specialist nurses in 31 provinces using convenience sampling. Results Among the investigated specialist nurses,96.5% were engaged in clinical nursing care,and most of them were practicing specialized nursing care(97.7%). Specialist nurses participating in specialist consultation and nursing care in the outpatient departments accounted for 63.6% and 59.7% respectively. For the influencing factors,81.1% of the specialist nurses proposed insufficient manpower;56.4% proposed that the duties were not clear;47.6% proposed that they did not function based on competence;46.5% proposed that support from management was lacking. For the satisfaction of career development,51.5% expressed great satisfaction with the hospitals’ support for specialist nurses;37.7% were very satisfied with the opportunities offered by hospitals for retraining and learning;31% were satisfied with the opportunity of promotion;27.9% were satisfied with the salary and welfare given by hospitals. For the needed supports,84.6% proposed to improve the income of specialist nurses;73.4% needed opportunities to participate in specialist-related training;70.2% indicated that they needed a platform for their function;65.8% suggested that job positions were needed. Conclusion The main work of specialist nurses in China is clinical specialist nursing. Nursing teaching,nursing management and nursing research need to be further strengthened. Lacking of manpower,unclear responsibilities and unstratified usage are the main factors affecting the role of specialist nurses. The satisfaction of specialist nurses to career development is not high,and hospitals should provide all aspects of supports.

    Objective To evaluate the status quo and equity of nursing human resources allocation in Henan Province in 2017,and to provide decision support for optimizing the allocation of nursing human resources in Henan Province. Methods Descriptive analysis of the current status of registered nurses in Henan Province from 2013 to 2017 was performed with the data of Henan Statistical Yearbook and China Health Statistics Yearbook, and equity of nursing human resources allocation in Henan Province was conducted using the Lorenz curve and Gini coefficient. Results From 2013 to 2017 the number of registered nurses had increased largely,but registered nurses per thousand people,ratio of physicians to nurses,and bed-to-nurse ratio in Henan Province were lower than that of the national average,and the total number of registered nurses was insufficient. In terms of equity of nursing human resources allocation,the Gini coefficients according to the resident population and geographical area were 0.18 and 0.36,and allocation based on the resident population was better than on the regional area. In addition,nursing human resources were unbalanced in urban and rural areas and among different institutions. Conclusion Nursing human resources allocation in Henan Province has been improved,but the total amount of registered nurses is insufficient. Health authorities should further optimize the allocation of nursing human resources in Henan Province to balance nursing human resources between urban and rural areas and among different institutions.

    Objective To investigate the current situation of human resources allocation of midwives in China. Methods A questionnaire survey was conducted by the judgment sampling method and convenience sampling from December 2017 to November 2018,on the midwifery human resources in tertiary and secondary hospitals in the member institutions of China Maternal and Child Health Association,which covered 22 provinces/ municipalities/autonomous regions in China. Results A total of 1254 questionnaires were distributed and 1213 valid questionnaires were collected with a valid rate of 97%. The average bed occupancy rate of obstetrics in China was 90%. Averagely each hospital had 12 midwives with the age of 24 to 35 years old who had been working less than 10 years with junior professional titles. The average outside training rate of midwives was 12%. There were significant differences in the annual delivery volume,bed occupancy rate,the number of midwives and composition between hospitals of different levels and types(P<0.05). Conclusion The shortage of midwives in China has been alleviated and the educational level of midwives has been improved. However,the number of midwives in China is still insufficient and its internal composition is unreasonable. Different levels and types of midwifery institutions have a difference in resource utilization. It is of great necessity for government to further carry out midwifery education and training,and optimize the allocation of midwifery human resources so as to achieve rational use of midwifery resources.

    Objective To describe the turnover status of nurses in China in 2017 and provide decision-making evidence for the health administrators. Methods We extracted the hospitals basic information,nursing human resource data and turnover data from the National Database of Nursing Quality reported by 940 hospitals from 30 provinces of China,and performed statistical analysis. Results The nursing turnover rate was 2.15% in 2017. The turnover rates in the second and third quarter were relatively higher than other quarters. The turnover rate declined from the east area,west area,to the middle area,which were 2.35%,2.02% and 1.92% respectively. In different provinces,turnover rates ranged from 0.75% to 4.60% and the higher turnover rates were mainly distributed in the eastern China,where economy levels were relatively high. The majority of nurses who quitted their jobs had the junior title,worked less than 5 years and held associate degree. There were differences in turnover rate among hospitals with different characteristics,different quarters,different educational background,job titles,and working years(P<0.001). The turnover rate was significantly correlated with the degree of the hospital,nature,comprehensive hospital,region,nurse-bed ratio,nurse-patient ratio and the average care time in 24 hours for one hospitalized patient and the proportion of nurses with associate degree or lower degree(P<0.001). Conclusion Currently,the nurse human resource in China is relatively stable. But 3.09% of the investigated hospitals and private hospitals had a turnover rate higher than 10% which should come into notice. Educational levels of nursing workforce and the nursing sensitive quality indicators concerning manpower disposition should be paid close attention to decline the turnover rate.

    Objective To develop a flexible allocation management platform for nurses in hospital and explore its effects of application in order to effectively allocate nurses human resources. Methods A research and development group as well as an expert team both including IT specialists were set up. An allocation model of nurses overtime working with hourly pay in different wards was proposed,and a revised draft of platform design was formed. It was developed and tested by professional IT specialists before being put on line. According to the inclusive criteria,220 nurses,18 head nurses and 2 administrators were included and authorized to use the platform for the allocation of nurses among departments in this hospital. Results The platform was formed with App terminal and PC terminal,including 3 entrances:administrator entrance,nurse entrance and head nurse entrance. After six-month application,105 nurses worked over-timely through the platform with a total of 1,295 hours. There were significant differences in the time and success rate of shift allocation(P<0.05) before and after the use of platform. There was no difference in the rate of adverse nursing events,compared with it of the same period in the previous year(P>0.05). Conclusion The platform can achieve nurse allocation crossing departments based on the wishes of nurses,and it can also increase the supply of nursing services,improve management efficiency and share in-hospital nursing human resources.

    Objective To develop the Head Nurse’s Transformational Leadership Self-rating Scale and verify its reliability and validity. Methods The scale was initially developed through literature review,semi-structure interviews and Delphi consultation. A total of 210 head nurses from a tertiary hospital in Wuhan were selected to conduct a pilot test. Item analysis and exploratory factor analysis were used to screen items. A total of 922 nurses from 18 tertiary hospitals in Hubei Province were recruited to conduct the formal test. The reliability analysis and confirmatory factor analysis were conducted using SPSS and AMOS software. Results The Head Nurse’s Transformational Leadership Self-rating Scale contained 4 dimensions and 25 items. The 4 demensions explained 61.092% of the total variance. The Cronbach’s α coefficient of the scale was 0.940,and the split-half reliability coefficient was 0.895. The one-dimension/multi-factor model’s CMIN/DF was 23.251,GFI was 0.919,AGFI was 0.902,CFI was 0.910,TLI was 0.900,and RMSEA was 0.049. Conclusion The Head Nurse’s Transformational Leadership Self-rating Scale can be used to evaluate the head nurse’s transformational leadership in Chinese context with good reliability and validity.

    Objective To establish an expert consensus on discharge planning for hospitalized elderly patients. Methods Studies were retrieved,evaluated and collected by the evidence-based nursing method. Revelent recommendations and research results were selected to form the expert consensus draft. Experts were invited to complete 2 rounds of Delphi expert consultation and 2 rounds of expert meeting. Each item was revised and adjusted considering objective evidence and expert opinions. Results The positive coefficients of experts in 2 rounds of Delphi expert consultation were all 100%,and the degrees of expert authority were all 0.83. The average importance value of each item was higher than 3.5;the variation coefficent of each item was less than 0.25;the Kendall’s W were 0.234 and 0.253 respectively(all P<0.001). At last,experts agreed on 5 dimensions of discharge planning for elderly patients. Conclusion The consensus can not only provide guidance for the clinical practice,but also promote the development of the discharge planning for hospitalized elderly patients in China.

    Objective To investigate the effect of stimulation intervention in elderly patients with traumatic brain injury and coma. Methods A total of 92 elderly patients with traumatic brain injury and coma admitted to a Grade III Level I hospital in Nantong city from February 2017 to February 2019 were enrolled by convenient sampling. They were allocated into the experimental group and the control group with 46 cases in each group based on the time of admission and the random number table. The control group received routine intervention,while the experimental group received stimulatory intervention. The awake situation,neurological recovery before and after intervention,changes in activities of daily living and complication rate were compared between the two groups. Results The awake rates of the experimental group within 15 and 30 days were higher than those of the control group,and the GCS score and ADL score of the experimental group after intervention were higher than those of the control group. The differences were statistically significant(P<0.05). The coma time of the experimental group was shorter than that of the control group,and the total complication rate of the experimental group was lower than that of the control group,with a statistical difference(P<0.05). Conclusion Stimulation intervention can be used to accelerate wakefulness,improve neurological function,improve activities of daily living and reduce the incidence of complications in elderly patients with traumatic brain injury and coma.

    Construction of evaluation index system of health management needs for home-based care of the disabled elderly

    Objective To construct the evaluation index system of health management needs for home-based care of the disabled elderly under the background of combination of medical and health care,in order to scientifically and systematically assess the health needs of the elderly disabled,promote and improve the health support system for the disabled elderly. Methods Through literature review,semi-structured interviews and Delphi method,the index system for assessing the health management needs of the disabled elderly was established. Results The effective recovery rates of 2 rounds of expert consultation were 100% and 94.44% respectively. The expert opinions tended to be consistent. Finally,the index system of the health management needs assessment for home-based care of disabled elderly with 4 first-level indicators,9 second-level indicators and 36 third-level indicators was formed. Conclusion The constructed index system of health assessment needs for the disabled elderly is scientific,reasonable,practical and content-oriented,which can provide references for the formulation of the health service system for the disabled elderly.

    Quality evaluation of evidence-based guidelines for the management of frail elderly

    Objective To evaluate the evidence-based guidelines for the management of frail elderly at home and abroad,and analyze the characteristics of each guideline,so as to provide references for the management of frail elderly in China. Methods We conducted a systematic retrieval of the relevant guidelines on frailty management of the elderly published or updated since the database was established until October 2018(the deadline for retrieval),in authoritative domestic and foreign guide websites,Chinese and English databases and the website of the professional society of gerontology and medicine. The 2017 updated version of the clinical research and evaluation tool for quality evaluation of guidelines meeting inclusion and exclusion criteria was used to analyze the characteristics of the guidelines. Results A total of 3 relevant evidence-based guidelines at home and abroad were retrieved. The average standardized scores of quality evaluation for the 6 areas were 97.5% for scope and purpose,68.5% for participants,65.3% for rigor,95.7% for clarity,68.7% for application,and 61.1% for independence. For overall quality,1 was Grade A and 2 were Grade B. Conclusion There is still a lack of evidence-based guidelines for the management of frail elderly at home and abroad,and the quality of developed guidelines needs to be improved,and contents need to be refined. Local evidence-based guidelines in China should be developed to guide the implementation of management of frail elderly.

    Effectiveness of whole-body vibration training on balance ability of the elderly:a meta-analysis

    Objective To systematically evaluate the effectiveness of whole-body vibration(WBV) training on balance ability of the elderly. Methods We searched databases including PubMed,Embase,Web of Science,CINAHL,PEDro,Cochrane Library,CBM,CNKI,Wanfang,and VIP and selected all randomized controlled trials(RCTs) that reported the effects of WBV in the elderly. Meta-analysis was performed by RevMan 5.3. The retrieval time is from the establishment of the database to March 2019. Results A total of 25 RCTs involving 1 763 old people were finally included. Meta-analysis results showed that WBV improved dynamic balance,static balance and functional balance of the elderly,and reduced the rate of falls(P<0.05),but there was no statistically significant difference in improving the physical health and mental health of the elderly(P>0.05). Conclusion WBV was effective in improving dynamic balance,static balance and functional balance of the elderly,as well as reducing the incidence of falls,but the effect is not significant in improving physical health and mental health.

    Objective To explore and evaluate the effects of attention and interpretation therapy on resilience and anxiety and depression for patients with gastrointestinal cancer. Methods Totally 94 cases of gastrointestinal cancer patients from a tertiary hospital in Ningxia were assigned into the experimental group(n=47) and the control group(n=47),and 2 groups of patients were both given routine nursing. On the basis of this,the experimental group was given attention and interpretation therapy for 10 weeks,the control group was given progress relaxation exercises. At the end of the intervention,patients were assessed with the self-made patient training compliance scale. Before intervention,at the end of intervention and 3 months after intervention,the effects were evaluated by CD-RISC,SAS and CMQ. Results The training compliance of the experimental group was significantly higher than the control group(P<0.05). There were statistically significant differences in the score of resilience,anxiety and depression between 2 groups (P<0.05). There was statistical significance between the score of each dimension of the resilience scale and the total score of it in the 2 groups(P<0.05). The time effect,treatment effect and the interactive effect of time and treatment in the total score of CD-RISC and its each dimension were statistically significant(P<0.05). The total score of anxiety and depression was significantly different between the two groups(P<0.05). The time effect,treatment effect and the interactive effect of time and treatment in the total score of anxiety and depression had statistical significance(P<0.05). Conclusion Attention and interpretation therapy can effectively improve the resilience level of patients with gastrointestinal cancer and alleviate anxiety and depression.

    Investigation on cognition and demand of moist dressing in non-wound specialty nurses

    Objective To investigate cognition and demand of moist dressing in non-wound specialty nurses,and to provide references for hospitals to develop targeted training programs. Methods A self-designed questionnaire survey was conducted on 937 non-wound specialty nurses in 4 tertiary hospitals. Results The overall average scores of cognition and demand were(38.56±13.26) and(32.43±7.74),and the corresponding scoring rate were 56.71% and 81.08%. There was significant difference in knowledge of non-wounds specialty nurses from different age,professional title,length of practice,and whether being the leader of the department(P<0.05). Conclusion Non-wound specialist nurses have poor level of cognition and high level of demand for moist dressing. Hospital-related training should pay attention to systematic training of wounds and moist dressing knowledge for non-wound specialty nurses in order to update the wound concept,enrich teaching methods and improve the enthusiasm of non-wound specialist nurses.

    Objective To develop a Gout Patient Self-management Scale and to test its reliability and validity. Methods Based on self-management theory,literature review and semi-structured interview,the provisional Gout Patient Self-management Assessment Scale was formed by Delphi technique and pilot investigation. Totally 260 gout patients were recruited to perform the item analysis for the provisional scale. Other 317 gout patients were investigated to test the reliability and validity of the final scale. Results The Gout Patient Self-management Assessment Scale consisted of 41 items in 4 dimensions. The Cronbach’s α coefficient of the total scale was 0.962;split-half reliability coefficient was 0.842;2-week test-retest reliability was 0.904. 4 common factors were extracted by exploratory analysis and the cumulative variance contribution rate was 60.779%. The content validity index(S-CVI) of the total scale was 0.905. Conclusion The Gout Patient Self-management Assessment Scale is a reliable and valid scale for measuring the self-management ability of gout patients.

    Objective To develop and implement an evidence-based nursing scheme for a patient with refractory skin ulcer of lower limb suffering from diabetes mellitus combined with essential thrombocytosis. Methods Using PICO principle,questions were put forward based on the clinical manifestations of the patient. We searched databases including UpToDate,Joanna Briggs Institute,Registered Nurses’ Association of Ontario,National Guideline Clearinghouse,National Institute for Health and Care Excellence,Scottish Intercollegiate Guidelines Network,Cochrane Library,CINAHL,Embase,Pubmed,CBM,WHO and websites of various professional associations. We included all clinical guidelines,evidence summaries,and systematic reviews on chronic wound care. After evaluating the feasibility,suitability,clinical significance and effectiveness of the evidence,we applied them to the patient and observed its effect. Results A total of 15 studies were finally included,including 3 clinical decisions,5 guidelines,1evidence summary,and 6 systematic reviews. We have formulated a targeted nursing program for the patient. On the basis of traditional treatment and nursing care,the specific program was developed and combined with the willingness of the patient,including no stopping use of hydroxyurea; Wagner grading system was utilized to grade and assess the wound comprehensively; cleaning the wound with normal saline and Compound Cortex Phellodendri liquid coating; performing the first debridement by ultrasonic debridement and adjusting the debridement method in time with the results of the wound assessment according to the wound condition; selecting the suitable dressing and combining with systemic treatment. After 16 days of active treatment,the wound infection of the patient was controlled. There was no obvious secretion,no self-reported pain and the granulation tissue grew well. The wound area was reduced and the symptoms of dry skin around the wound disappeared. The symptoms were improved and the patient was discharged. Conclusions For patients with diabetes mellitus combined with essential thrombocytosis,we can give them targeted,scientific and effective nursing measures through evidence-based methods to reduce the pain of patients,shorten wound healing time and improve the quality of life of patients.

    Objective To investigate the clinical safety of pulling peripheral venous indwelling needle according to clinical indications,and compare its effect with conventional indwelling needle extraction. Methods A multicenter randomized controlled trial was conducted with a total of 3,669 patients in 12 tertiary general hospitals from March 2019 to July 2019. The patients were randomly allocated into the observation group(group A) and the control group (group B) by the random number table. The puncture methods were consistent between the two groups. Researchers compared the indwelling time,the incidence of catheter-related complications within 96 hours,and the risk ratio of catheter-related complications during the entire indwelling period(phlebitis,occlusion,and infiltration,suspected infection) between two groups,among which phlebitis was the primary outcome indicator and other complications as secondary outcome measures. Results A total of 3,642 patients were enrolled in the study with 1,803 cases in group A and 1,839 cases in group B. The indwelling time of group A was(85±52)h and it was longer than that of (71±30) h in group B with a statistical difference(P<0.001). Within 96 h of indwelling time,there was no statistical differences in the risk of phlebitis,infiltration and suspected infection between two groups(P>0.05). During the entire indwelling period,the risk of occlusion in group A was higher than that in group B with a statistical difference(P<0.05). The other indicators showed no statistical difference. Conclusion The placement of indwelling needle based on clinical indications can prolong the indwelling time without increasing the risk of phlebitis,infiltration and suspected infection,but the risk of occlusion will increase. If the catheter is removed according to clinical indications,monitoring and evaluation of occlusion should be strengthened.

    Objective To systematically review and synthesize qualitative studies on the effects of environmental factors on functional activities and social participation of patients with stroke at home,in order to provide evidence for community health workers to improve home environment of stroke patients and to enhance their functional activities and social participation. Methods We searched databases including PubMed,Embase,Web of Science,PsycINFO,CINAHL,CBM and CNKI from inception to January 2019,to collect qualitative studies exploring environmental factors affecting stroke patients’ activities and participation. The quality of included studies was evaluated according to JBI Critical Appraisal Tool for qualitative studies in Australia. Meta-synthesis was used to integrate results. Results A total of 12 studies were included,and 34 findings were grouped according to their similarities to form 9 categories. Integration results 1:obstacles in living environment directly restricted patients’ activities and social participation;integration results 2:social support was crucial;integration results 3:the existing transportation services and health care services were difficult to meet patients’ needs. Conclusion Barrier-free environment plays an important role in the rehabilitation process of stroke patients at home. Government,community medical and health institutions and other relevant personnel should pay full attention to the impact of environmental factors on stroke patients at home,and help them establish a good social support network. Community health care services should be improved,and supportive organizations for stroke patients should be established to meet the needs of stroke patients at home.

    Prevalence and risk factors of subsyndromal delirium among adult intensive care unit patients:a systematic review

    Objective To determine the prevalence and risk factors of subsyndromal delirium (SSD) in adult intensive care unit patients by systematically reviewing evidence. Methods PubMed,Embase,PsycINFO,Cochrane Library,CBM,CNKI,VIP and Wanfang were searched from inception to 2019 for potentially relevant articles. Stata 14.0 software was used to analyze the prevalence of subsyndromal delirium with a single group meta-analysis. Fixed-effect or random-effect models were used to merge data. Begg rank correlation and funnel plot were used to determine publication bias. Descriptive analysis was used to analyze the influencing factors. Results 13 studies(5396 ICU patients) were included. The combined prevalence of subsyndromal delirium was 36.9%[95%CI(0.22~0.52)]. Subgroup analysis showed that the prevalence of subsyndromal syndrome was higher in women,elderly ICU and patients Asia. Possible risk factors included age,severity of diseases,cognitive impairment,and drug sedation. Conclusion There is a certain prevalence of subsyndromal delirium in ICU patients(36.9%). Age,underlying diseases,cognitive impairment,and drug sedation are possible risk factors. However,the overall sample size of the study is not large and the quality of the study is quite poor. It is suggested that large-sample multi-centered studies should be carried out in future to further explore the prevalence and risk factors of subsyndromal delirium in ICU patients.

    Translation and validation of the Chinese version of PEmb-QoL questionnaire

    Objective To develop the Chinese version of PEmb-QoL questionnaire and examine its reliability and validity. Methods According to the Brislin’s translation model,we followed the forward-translation and back-translation procedure to translate the PEmb-QoL questionnaire into Chinese language. The Chinese version was revised through experts’ review and a pilot study. The content validity of the questionnaire was evaluated by experts. Reliability and validity of the questionnaire were achieved through investigation among 201 patients with pulmonary embolism hospitalized and discharged in respiratory departments. Results The test-retest reliability of the questionnaire was 0.83. The Cronbach’s α was 0.85;the universal agreement scale-level content validity index was 0.94;the average scale-level content validity index was 0.96. 5 factors were extracted by exploratory factor analysis,which contributed to 76.65% of the variance. Conclusion The Chinese version of PEmb-QoL Questionnaire was proved to be a good instrument with acceptable reliability and validity,and it is qualified to assess quality of life among Chinese patients with pulmonary embolism.
