微波复合理疗对乳腺癌患者术后淋巴水肿干预效果的研究

摘要: 目的 探讨乳腺癌术后淋巴水肿的有效康复治疗护理方案。 方法 选取乳腺癌患肢淋巴水肿患者 94 例,随机分为干预组( 48 例)和对照组( 46 例),干预组给予微波理疗联合空气波压力泵和系统康复运动的复合理疗方法, 对照组给予空气波压力泵和系统康复运动的复合理疗方法,分别在干预前 1d 、干预后 2 、 4 、 6 个月比较两组患肢周径、肩关节活动度及生活质量等指标。 结果 重复测量方差分析显示,两组患肢周径的组间效应和肩关节外展程度的时间效应差异有统计学意义( P<0.05 );两组在生活质量的 5 个维度及生活质量总分的组间效应及时间效应差异均有统计学意义( P<0.05 ),其中情感状况维度得分的干预与时间因素存在交互效应( P<0.05 )。 结论 微波复合理疗对乳腺癌术后患肢淋巴水肿的康复治疗更加有效,适合应用于临床实践。

Abstract: Objective To explore an effective rehabilitation approach for breast cancer patients with lymphedema after the surgery. Methods A total of 94 breast cancer patients with lymphedema after the surgery were selected and divided into two groups randomly. In the intervention group ( 48 cases ), Microwave Therapy ( MT ) combined with Intermittent Pneumatic Compression ( IPC ) pump and Systematic Rehabilitation Exercise ( SRE ) were provided. In the
control group ( 46 cases ), the patients were treated with IPC combined with SRE. The involved upper arm circumference , the range of shoulder motion , level of swelling , and the quality of life were measured at one day before the intervention , and 2 , 4 , and 6 months after the intervention respectively in two groups. Results Repeated Measures Analysis of Variance showed that the differences between the two groups in the main effect on the involved upper arm circumference ( P<0.05 ), in the main effect of time on the range of shoulder outreach ( P<0.05 ), in the main effects of intervention and time on five dimensions and total scores of patients QOL ( P<0.05 ) were all statistically significant. The result also demonstrated that the score of emotion was associated with the time factor , indicating that they interacted with each other ( P<0.05 ) . Conclusion MT combined with IPP and SRE are more effective on the rehabilitation for breast cancer patients with lymphedema after the surgery and is recommended to clinical practice.
