湖南省二三级医院护士长循证实践领导力的现状调查

Implementation leadership of head nurses in Chinese tertiary and secondary hospitals:a cross-sectional study in Hunan

摘要:

目的 调查并分析湖南省二、三级医院护士长在循证实践中的实施领导力现状,为制订护士长实施领导力培训方案提供参考。方法 2020年10月—12月,通过多阶段整群随机抽样抽取湖南省5所三级医院和5所二级医院,每所医院随机抽取10个科室的护士长和护士为调查对象,采用实施领导力量表对护士长的领导力水平进行调查,使用SPSS 25.0进行数据统计描述和推断。结果 湖南省二、三级医院护士长实施领导力条目均分自评得分为(2.85±1.15)分,他评得分为(3.23±0.96)分。护士长实施领导力的支持性领导力得分自评和他评得分均为最高,知识性领导力在自评中得分最低,主动性领导力在他评中得分最低,主动性领导力、知识性领导力、持久性领导力3个维度以及总体他评得分均高于自评(P<0.05)。结论 湖南省二、三级医院护士长实施领导力有所提升,其中知识性领导力需要护理教育者和科研工作者重点关注。管理者应该为循证实践的发展提供开放、支持的环境,促进护士长实施领导力的提升,最终优化临床循证实践。

Abstract:

Objectives To investigate and analyze the status quo of head nurses’ implementation leadership in tertiary and secondary hospitals in Hunan Province,so as to provide references for the formulation of implementation leadership training programs. Methods From October to December 2020,2 cluster random sampling methods were used to sample 5 tertiary hospitals and 5 secondary hospitals in Hunan Province,with head nurses and nurses from 10 departments in each hospital,using implementation leadership scale(ILS) to investigate the leadership of head nurses in evidence-based practice. SPSS 25.0 was used for statistical description and inference of data. Results The score of implementation leadership of head nurses is(2.85±1.15) in self-evaluation and(3.23±0.96) in observer evaluation. Supportive leadership scores are the highest in both self-evaluation and observer evaluations;knowledge leadership scores are the lowest in self-evaluation;proactive leadership scores are the lowest in observer evaluations. The 3 dimensions of proactive leadership,knowledge-based leadership,and enduring leadership,as well as the overall scores of observer evaluation,are all higher than those of self-evaluation(P<0.05). Conclusion The implementation leadership of head nurse needs to be further improved,and its knowledge leadership is particularly important,which needs the attention of nursing educators and researchers. Managers should provide an open and supportive environment for the development of evidence-based practice,promote the implemen-tation leadership of head nurses,and finally optimize the implementation of clinical evidence-based practice.
