Qualitative study on transition readiness dilemmas of adolescents with inflammatory bowel disease towards adulthood

Objective To understand the dilemmas experience of adolescents with inflammatory bowel disease (IBD) as they transit towards adulthood,and to provide the basis for the development of patient-centered transition nursing intervention program. Methods With the application of phenomenological research method in qualitative research and purposive sampling method,from September to December 2022,15 adolescents with IBD from the department of gastroenterology in 2 tertiary A hospitals in Shanghai for semi-structured in-depth interview,and the interview data were analyzed by Colaizzi 7-step analysis method. Results 4 themes were extracted. ①Self-coping dilemma,including difficulty in adapting to changes in the medical environment,difficulty in adapting to changes in medical roles,difficulty in coping with negative emotional distress,difficulty in meeting the special needs of daily life. ②Family support dilemma,including heavy family financial burden,excessive participation of family caregivers,and insufficient family resilience. ③Medical assistance dilemma,including limited professional assistance and limited information sharing. ④Social support dilemma,including the limited applicability and accessibility of social support,insufficient social cognition and identity. Conclusion Adolescents with IBD face multiple dilemmas as they move towards adulthood. It is suggested that nursing staff should formulate a perfect personalized transition plan,carry out transition-oriented family health education,improve the medical assistance system,further strengthen social support,and improve the level of transition readiness for the adolescents towards adults.

The transition period from children with inflammatory bowel disease(IBD) to adults is an important stage that affects their long-term quality of life. However,most children with IBD have a low degree of readiness during the transition period,which seriously affects the prognosis and quality of life of children. This paper reviews the research progress on the domestic and foreign status of transition readiness for children with IBD,influencing factors,defects and intervention strategies. Suggestions,shortcomings in current research and practice are put forward,aiming to carry out relevant research for our country in the future and provide references to improve the transition readiness of children with IBD to adults.

Development of a simple judgment card for classification of nursing care level for home-stayed stroke patients

Objective To develop a simple judgment card for nursing care level classification for home-stayed stroke patients,in order to lay the foundations for nurses in the community health service centers to provide professional care services for home-stayed stroke patients. Methods Nursing classification evaluation indicator system for home-stayed stroke patients was adopted by the research group to comprehensively evaluate 210 stroke patients. The analytic hierarchy process and weighted rank sum ratio method were used to explore the classification of nursing grades in stroke patients. Results The regression equation constructed by the weighted rank sum ratio method is Y=0.807 1-0.060 8 × probit. 66 cases were classified as the first-level nursing care,31 in the second-level nursing care,32 in the third-level nursing care,and 81 in the fourth-level nursing care. Conclusion The simple judgment card for classification of nursing care level for home stayed stroke patients is science-based and convenient,and it has strong practical significance in reasonable allocation of community health human resources to meet the professional care needs of home-stayed stroke patients.

Intervention research of undergraduate intern nursing students’ professional identity

Objective To explore the influence of cognitive intervention program on undergraduate intern nursing students’ professional identity and professional self-efficacy.Methods Two comprehensive tertiary class-A hospitals which were comparable in scale,salary,hospital environment and quality of teaching staff,were selected using convenience sampling method,and were randomly assigned as the experimental group and the control group. Convenience sampling method was used to recruit 75 nursing students in the experimental group and 69 nursing students in the control group. The intervention program was based on ABC Theory and Broaden-and-Build Theory. The experimental group was given a 5-times cognitive intervention training biweekly based on the routine training. The intervention program mainly consisted of the online and offline lectures and the interactive discussion in class. The control group was given a 5-times routine professional learning during intern practice.Results A total of 138 nursing students completed the study. The overall score of nursing students’ professional identity and professional self-efficacy in the experimental group was better than those in the control group and the differences were statistically significant after the intervention(P<0.05).Conclusion The intervention program based on ABC Theory and Broaden-and-Build Theory can improve intern nursing students’ professional identity and professional self-efficacy effectively.
