脑卒中后失语症患者筛查和护理的最佳证据总结

摘要:

目的 检索、分析和汇总脑卒中后失语症患者筛查和护理的相关证据,促进脑卒中后失语症患者的准确筛查和有效管理。 方法 2019年12月,依照证据金字塔模型,计算机检索相关文献,采用乔安娜布里格斯研究所(Joanna Briggs Institute,JBI)循证卫生保健中心对应的文献评价标准和证据分级系统对文献进行质量评价,并对提取的证据进行等级划分。检索时限为2010年1月1日—2019年12月30日。 结果 共纳入文献12篇,包括指南4篇、最佳实践2篇、临床决策1篇和系统评价5篇,从脑卒中后失语症患者的评估、诊断、计划、实施和评价5方面总结出24条证据。 结论 脑卒中后失语症患者筛查和护理的证据为有效促进患者康复提供循证依据,建议在应用证据时考虑我国文化特点、医疗资源配备和患者自身状况等选择针对性的证据。

Abstract:

Objective To retrieve,appraise and integrate the available evidence on aphasia in patients with stroke,so as to promote the accuracy of recognition and the efficiency of management. Methods Based on the model of evidence pyramid,relevant literature published in the past 10 years were searched and retrieved. JBI evidence appraisal and recommendation system were used to evaluate quality of studies and the level of evidence.Results A total of 12 papers were included,including 4 guidelines,2 best practices,l clinical decision and 5 systematic reviews. Totally 24 pieces of best evidence were summarized from 5 aspects of assessment,diagnosis,planning,implementation,and evaluation. Conclusion The screening of post-stroke aphasia facilitates early recognition,and the evidence of post-stroke aphasia management provides evidence-based references for the rehabilitation of patients with post-stroke aphasia. It is recommended that cultural characteristics,medical resources and the patient's own condition should be taken into consideration while applying the evidence.
