心血管疾病患者远程康复体验质性研究的Meta整合

Meta-synthesis of qualitative research on the real experience of telerehabilitation for patients with cardiovascular diseases

摘要:

目的 系统评价、整合心血管疾病患者远程康复体验及需求的质性研究,为临床应用及改善远程康复技术提供参考依据。 方法 计算机检索Cochrane Library、澳大利亚乔安娜布里格斯研究所(Joanna Briggs Institute,JBI)循证卫生保健中心图书馆、PubMed、Embase、Web of Science、PsycINFO、CINAHL、中国生物医学数据库、万方数据库、中国知网和维普数据库,检索关于心血管疾病患者对于远程康复体验及需求的质性研究,检索时限均为从建库至2023年2月。采用澳大利亚JBI循证卫生保健中心2016版质性研究质量评价标准进行文献质量评价,采用汇集性整合方法对结果进行归纳、整合。 结果 共纳入15篇文献,提炼出65个明确的研究结果,将相似结果归纳、整合,形成11个新的类别,并综合成4个整合结果:心血管疾病患者对远程康复的接受度;心血管疾病患者远程康复的感知益处;心血管疾病患者远程康复的障碍因素;心血管疾病患者对远程康复的需求与期望。 结论 远程康复作为传统心脏康复的便捷替代方案,其效果得到多数心血管疾病患者的肯定,但未来研究应不断优化数字工具,完善康复模式,加强团队建设,提供社会支持,提升心血管疾病患者使用远程康复系统的可及性和体验。

Abstract:

Objective To systematically evaluate and integrate qualitative studies on the experience and needs of telerehabilitation for patients with cardiovascular diseases,so as to provide references for clinical practice in China. Methods We searched databases including Cochrane Library,JBI,PubMed,Embase,Web of Science,Psyc INFO,CINAHL,CBM,WanFang Data,CNKI and VIP from inception to February 2023,to collect qualitative studies related to the experience and needs of telerehabilitation for patients with cardiovascular diseases. The quality of included studies was evaluated according to JBI Critical Appraisal Tool for qualitative studies in Australia. Meta-synthesis was used to integrate results. Results A total of 15 studies were included;65 findings were collected;the similar results were grouped into 11 new categories;finally these categories were integrated into 4 synthesis results,including acceptance of telerehabilitation in patients with cardiovascular disease,perceived benefits of telerehabilitation in patients with cardiovascular disease,barriers to telerehabilitation in patients with cardiovascular diseases,telerehabilitation needs and expectations of patients with cardiovascular disease. Conclusion As a convenient alternative to telerehabilitation additional cardiac rehabilitation,telerehabilitation has been recognized by most patients with cardiovascular diseases. However,future research should continue to optimize digital tools,improve rehabilitation models,strengthen team building,provide social support,and improve the accessibility and experience of cardiovascular disease patients using telerehabilitation systems.
