Cognitive adaptation training can bypass the cognitive deficits of patients with severe mental illness (SMI),use personalized compensation strategies and environment to support the development of patients’ adaptive behaviors, and improve their social function, medication adherence, quality of life and prognosis. In recent years,nurse-led cognitive adaptation training has developed into a mature nursing intervention after feasibility exploration and continuous improvement of research methods abroad, and it has been integrated into psychiatric wards as routine nursing. This paper reviews the application status, intervention contents and intervention effects of cognitive adaptation training in patients with SMI,and puts forward suggestions on cognitive adaptation training in psychiatric wards and community environments in China from 4 aspects,in order to provide references for the research and clinical practice on cognitive adaptation training in China.
Objective To describe the scope and breadth of primary research regarding the implementation of the Safewards Model and its interventions in psychiatric wards. Methods A comprehensive search of the electronic databases, such as PubMed,Embase,CINAHL,PsycINFO,Web of Science,Scopus,Cochrane Library,JBI Evidence-Based Practice,JBI Evidence Synthesis,CNKI,WANFANG,VIP,and SinoMed,from database inception to August 2022,has been conducted to retrieve all the potentially relevant articles. 2 researchers independently screened and extracted information for the retrieved literature, and the evidence was then synthesized and summarized. Results The included 19 articles comprehensively evaluated 14 safewards implementation studies in terms of its effectiveness, intervention fidelity, and feasibility. Conclusion The safewards interventions,if implemented with high fidelity, can effectively reduce the incidence of conflict events in psychiatric wards and the use of restrictive practices, and improve the ward atmosphere. Safewards provide an opportunity and platform by which trauma-informed and recovery-oriented practices in psychiatric setting can be achieved. For the future implementation of safewards,it is necessary to strengthen the training of staff,to help staff develop positive attitudes and beliefs toward safewards, and to seek support from hospital management level.
To summarize the weaning nursing practice of a patient with postoperative cardiogenic shock complicated by acute type A aortic dissection receiving veno-arterial extracorporeal membrane oxygenation. The key points of nursing were as follows:the interprofessional team cooperation model was adopted, and the extracorporeal membrane oxygenation special class team was established;a goal-oriented extracorporeal membrane oxygenation management plan was formulated and individualized pre-weaning nursing was implemented. The evaluation and preparation before weaning should be improved, and the cooperation during weaning and the observation after weaning should be strengthened. After careful treatment and nursing, extracorporeal membrane oxygenation was successfully removed on the 7th day after operation, and the patient recovered and was discharged on the 10th day after surgery, and the follow-up was in good condition.
At present, the quality of life of patients with bipolar disorder(BD) is relati-vely low,and because of the special nature of their lack of insight, few nursing interventions have been carried out in China for them. This article reviews the latest studies on quality of life in patients with BD,including the current status of their quality of life, assessment tools, and nursing interventions to improve quality of life. It is recommended that evidence-based nursing interventions should be developed and implemented in clinical practice and community setting. The effective nursing interventions that are suitable for China’s clinical reality and can improve the BD patients quality of life should be further explored,so as to provide bases for the construction of systematic intervention programs.
Objective To systematically evaluate the disease experience of patients with schizophrenia in rehabilitation period, and to provide the evidence for strengthening standardized management in rehabilitation period and improving the rehabilitation effect and mental health level of this population. Methods A computer search was conducted on PubMed,Web of Science,Cochrane Library,CINAHL,Embase,PsyINFO,CNKI,Wanfang Database,VIP Science and Technology Journal Database and China Biomedical Literature Database for qualitative research on mental experience, real experience and feelings of schizophrenia patients in convalescence. The search time was from the establishment to February 2022. The literature quality evaluation was carried out by the quality evaluation standard of qualitative research of Joanna Briggs Institute(JBI),Australia(2016 edition),and the research results were summarized and integrated by the convergent integration method. Results Totally 13 articles were included;86 topics were extracted;10 categories were summarized. 4 integrated results were formed as following. Schizophrenia is a complex mystery, which is often accompanied by many negative disease experiences;social isolation and alienation from the outside world;the attempt to rebuild the life;rehabilitation support needs to be completed together. Conclusion Medical staff should pay attention to the emotional experience of patients, fully explore the positive feelings, meet the reasonable needs of patients, and give patients systematic rehabilitation support. The public should reduce the stigma of patients and create a good social environment.
Objective To explore the relationship between mental pain,self-compassion and automatic thoughts in depressed patients. Methods A total of 263 depressed inpatients were selected and tested by the Chinese version of the Orbach & Mikulincer Mental Pain Scale(OMMP),Self-Compassion Scale-Chinese Version(SCS-C),Automatic Thought Questionnaire(ATQ) between 2020 and 2021 from a tertiary psychiatric hospital through convenient sampling. SPSS 25.0 was employed for Pearson correlation analysis and multiple linear regression analysis, and we selected the Model 4 of SPSS PROCESS for mediating effect analysis. Results Mental pain was significantly positively correlated with automatic thoughts(P<0.001) and negatively associated with self-compassion(P<0.001). The results of Bootstrap test showed that the mediating effect of automatic thoughts on the relationship between self-compassion and mental pain was -0.274(95%CI:-0.356~-0.203),accounting for 59.6% of the total effect. Conclusion The automatic thoughts play a partial mediating role on the relationship between self-compassion and mental pain in depressed patients. Based on this path of mental pain, nurses should actively constitute intervention plans,in order to achieve the final goal of decreasing the incidence rate of suicide or self-injury in patients with depression.
