Narrative intervention for patients living with breast cancer:a scoping review

Objective The scope of literature review correlated to narrative intervention for patients living with breast cancer was conducted to define the object,form,content,timing/duration and patient outcomes. It can provide references for future research and development of efficient,scientific,and rigorous narrative intervention programs for breast cancer patients and guide clinical practice. Methods According to the scoping review methodology,we systematically searched PubMed,CINAHL,Embase,Web of Science,Cochrane library,CNKI,WanFang database and VIP database. The search period is from the establishment of the database to December 31,2022. There are 2 researchers who analyzed the included studies independently. Results A total of 25 papers from 6 countries were included,and 12 of which coming from China. The results found that the narrative intervention runs throughout the treatment of breast cancer patients. Patients at different treatment stages have different needs for narrative intervention. Less attention had been paid to breast cancer survivors. The forms of narrative intervention include “telling story”,“writing story” and “watching/reading story”. Most of narrative practice forms were “telling story”. Overall,the narrative intervention had significant improvements in psychological,physical,cognitive and life quality,family relationship/social support for breast cancer patients. Conclusion Narrative intervention has positive effects on patients with breast cancer. Currently,the forms of narrative practice for patients with breast cancer need to be further enriched. More attention should be paid to breast cancer survivors,and narrative intervention programs should be individualized according to patient characteristics to enhance intervention effects.

With the development of Internet technology,micro-credentials,as an innovative competency-based and result-oriented education,are supplementary certification models. It is not only a flexible credentialing of nursing professional competencies,but also can promote the circulation of certificates. Micro-credentials can make up for the shortcomings of traditional education certification models and meet the continuing professional development of the nursing. We summarized the development of micro-credentials and its application in nursing,and further discussed its implications,in order to provide scientific evidence for promoting the application and development of micro-credentials in nursing.

Summary of the best evidence for exercise training in patients with peripheral artery disease

Objective To search,summarize and evaluate the best evidence of exercise training in patients with peripheral arterial disease(PAD),which could further provide evidence-based medical bases for clinical exercise training intervention in patients with PAD. Methods All evidence on exercise training in patients with PAD were retrieved from UpToDate,Society for Vascular Surgery(SVS) website,European Society for Vascular Surgery(ESVS) website,Cochrane Library,PubMed,Springer Link,Embase,Cumulative Index to Nursing and Allied Health Literature(CINAHL),Med-live website,Chinese Society of Vascular Surgery,the Chao-Xing journal database,China National Knowledge Infrastructure(CNKI),Wan-Fang database,and other Chinese and foreign technological journal databases. Evidence was obtained from guidelines,clinical decisions,expert consensuses,evidence summaries,systematic evaluation,and meta-analysis. The retrieval time limit was from the establishment of databases until July 1,2021. There were 2 researchers who evaluated the literature quality and extracted the data. Results A total of 14 articles were included in the present analysis. A total of 20 pieces of best evidence were extracted from 7 aspects,including exercise safety,exercise timing,exercise mode,exercise intensity,exercise time,effect evaluation,and tips. Conclusion The exercise training could improve the walking ability and the quality of life of PAD patients. Thereby,it is recommended that exercise training intervention should be conducted while ensuring safety. The evidence summary process of this study is rigorous and comprehensive. In clinical practice,the medical staff should consider the actual application environment and individual situation of patients when using the evidence.

Modification and effect evaluation of prediction of orogastric tube insertion length in premature infants

Objective To modify the insertion length of orogastric tube placement in premature infants and to evaluate its effect. Methods A total of 306 premature infants undergoing orogastric tube insertion in the Neonatal Center of a tertiary A hospital in Beijing were selected as the study subjects. 155 premature infants undergoing orogastric tube insertion from August 2019 to April 2021 were selected as the test group,and 151 premature infants from March 2018 to July 2019 were in the control group. The effects of different predicting methods on the correct placement rate of the orogastric tube tip,drainage volume before gastrointestinal decompression,number of vomiting episodes,and the total time of gastrointestinal decompression were compared in 2 groups. Results The correct placement rate of orogastric tube tip was 85.81% in the test group and 15.23% in the control group,and the difference was statistically significant(P<0.001). The drainage volume 3 days before gastrointestinal decompression in the test group was higher than that in the control group,and the difference was statistically significant(t=14.38,t=15.78,t=11.06,all,P<0.001). The incidence rate of vomiting during gastrointestinal decompression in the test group was lower than that in the control group(χ2=7.05),and the difference was statistically significant(P=0.008). The total time of gastrointestinal decompression was shorter than that in the control group(t=12.74),and the difference was statistically significant(P<0.001). Conclusion The modified method of predicting insertion length of orogastric tube in premature infants can improve the accuracy of optimal placement for the orogastric tube tip,avoid misplacement of the orogastric tube,and improve the effect of gastrointestinal decompression.

Risk factors of stroke and fear of stroke recurrence in elderly stroke patients with semi disability

Objective To investigate the current situation of fear of stroke recurrence in elderly stroke patients with semi disability,and to analyze its influencing factors. Methods A total of 584 stroke patients from 23 communities in Jinan City,Shandong Province from May to December 2020 were randomly investigated by the general information questionnaire,Fear of Progression Questionnaire-Short Form(FoP-Q-SF),Brief Illness Perception Questionnaire(BIPQ) and Barthel Index Scale. Results The level of fear of stroke recurrence was higher in elderly patients with stroke and semi disability. The score of fear of stroke recurrence in 584 elderly patients with stroke and semi disability was (38.89±7.13),among which 498 patients(85.27%) scored ≥34. The score of BIPQ was (50.12±7.65) and Barthel Index was (43.86±6.16). Multiple stepwise regression analysis showed that age,Barthel Index Score,course of disease,recurrence times and disease perception were the influencing factors of fear of stroke recurrence(P<0.05). Conclusion The fear of stroke recurrence is more common in elderly stroke patients with semi disability,and the incidence rate is relatively high. Medical staff should make intervention measures according to the influencing factors of the fear of stroke recurrence,so as to improve the cognition of stroke patients to the disease,improve the basic daily living ability of stroke patients,and reduce the fear of stroke recurrence in elderly stroke patients with semi disability.

Objective To formulate an expert consensus on diaper dermatitis nursing care for infants and toddlers,and to standardize the clinical nursing practice. Methods Literature searching/reviews and expert discussion meetings were used to formulate the main contents of the consensus. The consensus was then revised by 2 rounds of Delphi expert consultations and expert meetings. Results The final expert consensus on diaper dermatitis nursing care for infants and toddlers included 3 aspects of diaper dermatitis nursing care(evaluation,prevention/nursing care,treatment of infection),and a total of 31 items. Conclusion Items in expert consensus on diaper dermatitis nursing care for infants and toddlers are concise and easy to implement,and the consensus is helpful to guide clinical practice of diaper dermatitis nursing care.

Study on construction of non-pharmacological sleep intervention program for patients with shallow sedation in ICU

Objective To establish a sleep management program appropriate for ICU patients in China,so as to improve the sleep quality of ICU patients. Methods The preliminary sleep management strategies were designed and the related evidence was obtained by retrieval of both domestic and oversea databases. The appraisal of literature was conducted by Jonna Briggs Institute 2010 evidence grade and quality evaluation methods. Then the Delphi method was adopted to make the strategies meet our national conditions. Results 30 sleep management strategies were established under 11 dimensions and 2 parts including “preconditions for the implementation of a non-pharmacological sleep intervention program for patients with shallow sedation in ICU” and“the constitution and implementation of a non-pharmacological sleep intervention program for patients with shallow sedation in ICU”. Conclusion The non-pharmacological sleep intervention program for ICU patients with shallow sedation constructed on the basis of evidence-based and Delphi method is scientific and practical,and it can be applied for sleep management of ICU patients.
