Evidence summary for maternal postpartum management of gestational diabetes mellitus

Objective To provide solid evidence for clinical development and implementation of maternal postpartum management of gestational diabetes mellitus,we systematically retrieve,extract,summarize,and analyze the latest and the best evidence of maternal postpartum management of gestational diabetes mellitus. Methods According to the top-down principle of the evidence-based resource 6S model,a systematic search of domestic and foreign computer decision support systems,guideline websites,databases,and relevant social websites was conducted for evidence on maternal postpartum management of gestational diabetes mellitus,including clinical practice guidelines,best clinical practice information manuals,evidence summaries,expert consensuses,systematic reviews,and standards. The search time limit was from the establishment of the databases to April 2023. 2 trained researchers in systematic evidence-based knowledge evaluated the quality of the literature,combined with professional judgment,selected the literature that met the inclusion and exclusion criteria,and extracted the evidence. Results A total of 16 articles were included,including 9 guidelines,5 systematic reviews,and 2 expert consensus articles. The best evidence includes 32 recommendations in 6 aspects:breastfeeding,postpartum medication precautions,lifestyle,postpartum follow-up and screening,contraception,and repeat pregnancy. Conclusion The study summarizes the best evidence for postpartum management of women with gestational diabetes,and provides an evidence-based basis for targeted application of evidence by clinical healthcare providers.

The application of mobile blood glucose management in pregnant women with gestational diabetes:a scoping review

Objective A scoping review of research on mobile glucose management in pregnant women with gestational diabetes mellitus was conducted to provide references for clinical providers to conduct mobile glucose management and related research. Methods Based on Arksey and O’Malley’s scoping review reporting framework,JBI Library,Cochrane Library,PubMed,Web of Science,Embase,CINAHL Complete,CBM,China National Knowledge In-frastructure(CNKI),and Wanfang Database were retrieved. The search time frame was from the establishment of the databases to February 14,2023. The included literature was screened,summarized and analyzed. Results A total of 65 publications from 11 countries were included,among which 38 were randomized controlled trials,27 were experimental studies. The vehicles relied on for mobile glucose management in gestational diabetes mainly included APPs,remote monitoring systems,professional websites,and social platforms,which could provide health education,glucose management monitoring and recording,social support,and glucose management follow-up and reminders to pregnant women. Most of the studies focus on the glucose index and drug use,pregnancy outcome,self-management,quality of life,user experience and resource cost of pregnant women. Conclusion Mobile glucose management carriers are geographically specific and have a positive effect on glucose control in gestational diabetes. Future studies need to further develop localized mobile glucose management intervention protocols and conduct more high-quality randomized controlled studies to verify their effectiveness.

Textual analysis of China’s nursing and health policies from the perspective of policy instruments

Objective To study the use of policy instruments in nursing and health policies from 1978 to 2020,and to explore the distribution and framework of existing policy instruments,so as to provide references for the formulation and implementation of subsequent policies. Methods Based on Rothwell & Zegveld’s theory of policy instruments,this paper uses the content analysis method to construct a two-dimensional analysis framework of dimension X(basic policy instrument) and dimension Y(policy development stage),collating and quantifying the existing nursing health policy texts. Results 24 policy documents were finally included,and a total of 378 policy items were coded. In the X dimension,supply-oriented,demand-oriented,and environmental-oriented policy instruments accounted for 24.60%,14.02%,and 61.38%,respectively. In the Y dimension,the policy instruments in the budding exploration period,the supplementary improvement period,and the mature promotion period account for 23.02%,31.75%,and 45.24%,respectively. Conclusion At present,the overall use of nursing and health policy instruments in China is not coordinated. Supply-oriented policy instruments pay more attention to personnel training,and scientific and technological research and information construction need to be increased. Demand-oriented policy instruments are less applied and lack effective pulling effect. Environmental policy instruments are used too frequently and the internal ratio needs to be adjusted. It is suggested to further grasp the evolution rules of nursing health policy,improve the effect of policy implementation,and strengthen the integrity of policy.

The best evidence summary of exercise management of patients with nonalcoholic fatty liver disease

Objective To search and obtain relevant evidence of exercise management in patients with nonalcoholic fatty liver disease(NAFLD) and to provide a reference for clinical practice. Methods UpToDate,BMJ Best Practice,International Guidelines Collaboration(GIN),Scottish College Guidelines Network(SIGN),National Institute for Healthcare Excellence(NICE) website,Cochrane Library,U.S. Agency for Healthcare Research and Quality(AHRQ) website,Canadian Association of Registered Nurses of Ontario(RNAO) website,American Association for the Study of Liver Diseases(AASLD) website,PubMed,Web of Science,CINAHL,DynaMed,Embase,CNKI,Chinese Medical journal full-text database,Wanfang Data Knowledge Service Platform,Sinomed and other databases were systematically searched for all evidence on exercise management of the NAFLD. It includes guidelines,evidence summaries,expert consensuses,best practice information booklets,clinical decisions,recommended practice,and systematic reviews. The retrieval period is from the database inception to November 2022. The quality of the literature was evaluated independently by 2 researchers. Results A total of 18 pieces of literature,including systematic evaluations,expert consensuses,and clinical practice guidelines,were included. 24 pieces of best evidence were collected from 6 aspects,including exercise principle,exercise supervision,exercise type,exercise selection,exercise time and frequency,and exercise intensity. Conclusion This study summarized the best evidence for exercise management of patients with NAFLD,which is comprehensive and scientific. Medical staff can use the best evidence to guide patients with NAFLD to exercise reasonably and improve the scientific nature of the intervention.

The reliability and validity test of the Chinese version of Perioperative Thirst Discomfort Scale

Objective To translate the English version of Perioperative Thirst Discomfort Scale (PTDS) into Chinese,and to test the reliability and validity of the Chinese version of PTDS. Methods The English version of PTDS was translated into Chinese and back translated and modified for cultural adaptation according to the guidelines. A convenience sample of 316 perioperative patients at 4 tertiary hospitals in Fujian was investigated by the Chinese version of PTDS,in order to test its reliability and validity. Results The Cronbach’s α of the Chinese PTDS was 0.813; both the scale-level content validity index and the item-level content validity index were 1.Factor analysis showed 2 factors explaining 64.312% of the total variance. The score of PTDS was significantly associated with the thirst intensity score(r=0.681,P<0.001). Conclusion The Chinese version of PTDS has been proved to be reliable and valid.It can be used for measuring thirst discomfort in perioperative patients.

The construction and application of intelligent information management system for donor human milk bank

Objective To construct an intelligent information management system of donor human milk bank,to establish a management information platform connecting of human milk donors,managers and recipients,to standardize and optimize the process,and to explore suitable human milk bank management methods suitable for China's national conditions. Methods From October 2017 to November 2019,the information management system of human milk bank was constructed including the following 5 functional modules,namely importing donor information,inputting milk information,managing donors' milk in and out,tracking adverse events,statistical analysis. The staff efficiency,the number of error events and breastfeeding rates were compared between July to August 2019 before the system application and October to December 2019 after the system application. Results Before and after the application of the management system of the donor human milk bank,the average operation time of the donor milk input for storage was reduced from 8min/portion to 2min/portion,and the average operation time of the donor milk out from the storage was reduced from 5min/portion to 1min/portion. The daily calculation of milk consumption by the staff was also changed from 1h daily to be automatically generated by the system. The backtracking time of milk source information was changed from 1.5h/person before application to automatic system correlation. The incidence of adverse events due to mis-delivery or near shelf life of donor milk decreased from 3 cases before application to 0 after application. The breastfeeding rate before and after system application increased from 65.11% to 69.74%,and the difference was statistically significant(χ 2=12.55,P<0.001). Conclusion The intelligent information management system of human milk bank can help health care professionals to optimize the operation process,ensure the safety of donor human milk breastfeeding,and improve the breastfeeding rate in neonatal intensive care unit.
