Research progress on rehabilitation motivation assessment tools for stroke patients

The rehabilitation compliance of stroke patients is generally low. Evaluating the rehabilitation motivation of patients is helpful to promote the rehabilitation management of patients,enhance the rehabilitation enthusiasm and compliance of patients,and improve the rehabilitation outcome. This paper reviews the existing stroke patients rehabilitation motivation assessment tools,and expounds the main contents,application status,characteristics and limitations of stroke patients rehabilitation motivation assessment tools,in order to provide references for the appropriate selection of clinical assessment tools,the rehabilitation management of stroke patients and the development of domestic localized stroke rehabilitation motivation assessment tools.

Construction of a practical program for interhospital transfer of critically ill children

Objective To develop an evidence-based,localized practice protocol for the interhospital transfer of critically ill children. Methods Through a comprehensive evidence summary and semi-structured interviews,a preliminary inter-hospital transfer practice protocol for critically ill children was formulated. A panel of 31 experts from 12 hospitals in China participated in 2 rounds of expert correspondence between May and July 2022,facilitating meticulous revision of the protocol entries. Results The response rate for both rounds of questionnaires was 100%,and the expert authority coefficients ranged from 0.926 to 0.931. In the second round of consultation,the coefficient of variation for the importance score of each entry ranged from 0.036 to 0.226,and the Kendall’s W was determined to be 0.201(P<0.001). Additionally,the coefficient of variation for the feasibility score of each entry fell within the range of 0.070 to 0.314,with Kendall’s W of 0.124(P<0.001). Ultimately,the final interhospital transfer protocol for critically ill children comprised 8 level Ⅰ entries,16 level Ⅱ entries,and 75 level Ⅲ entries. Conclusion The interhospital transfer protocol constructed in this study is grounded in scientific evidence and exhibits practical feasibility. It serves as a valuable reference for organizing and implementing interhospital transfers of critically ill children.

The illness experience of adolescent patients with systemic lupus erythematosus:a qualitative study

Objective To understand the illness experience of adolescent patients with systemic lupus erythematosus,so as to provide references for clinical medical staff to formulate targeted intervention strategies. Methods Semi-structured interviews were carried out from January to April 2022 by phenomenology. 11 adolescent patients with systemic lupus erythematosus from the pediatric clinic or ward of a tertiary A hospital in Beijing were recruited,and Colaizzi’s 7-step analysis method was applied to analyze and refine themes of interview data. Results 4 themes were extracted. Theme 1:bad physiological experience(pain and fatigue); theme 2:negative psychological experience(suffer harm from others,prejudice against oneself,fear of disease,and worries about the future); theme 3:disease coping style(conflict and resistance,compromise and concession,acceptance and adjustment); theme 4:po-sitive psychological experience(feel the support of others and gain self-growth). Conclusion Pediatric medical staff should pay attention to the physical and mental feelings of adolescent patients with systemic lupus erythematosus in the course of illness,and give scientific and effective interventions to promote their physical and mental comfort.

Intervention effects of rehabilitation training of mode of human occupation on self-efficacy and stigma of schizophrenia patients

Objective To explore the intervention effects of rehabilitation training of mode of human occupation (MOHO) on self-efficacy and stigma of convalescent schizophrenic patients. Methods 103 inpatients with schizophrenia were randomly divided into a research group(53 cases) and a control group(50 cases). The study group received a rehabilitation training of MOHO on the basis of anti-psychotic treatment and routine nursing, 5 times a week for 8 weeks. The control group received a routine rehabilitation training on the basis of anti-psychotic treatment and routine nursing. Before and after the intervention,the general self-efficacy scale(GSES) and the stigma scale for patients with mental illness(SSMI-C) were used to evaluate the effect of the intervention. Results The results of intra group comparison showed that GSES was(15.46±2.74) before intervention and(17.92 ±2.42) after intervention. Before the intervention,the total score of stigma was(52.12±7.53),discrimination was (21.36±6.83),disease concealment was (17.28±4.45),positive effect was(13.48±3.81);after the intervention,the total score of stigma was (41.66±4.95),discrimination was(16.84±4.02),disease concealment was (14.18±2.75),and positive effect was(10.64±2.38). The difference was statistically significant(P<0.001). The comparison results within the control group showed that the post test score of self-efficacy was higher than the pretest score,and there was no significant difference in GSES score,total score of stigma,discrimination,disease concealment and positive effect factor score(P>0.05). After the intervention,the GSES score of the research group was higher than that of the control group,and the total score and each dimension score of stigma were lower than those of the control group(P<0.001). Conclusion The rehabilitation training of MOHO can effectively reduce the level of stigma and improve the sense of self-efficacy.

Objective The establishment of an evaluation index for the quality of prone ventilation care for patients with acute respiratory distress syndrome (ARDS) is to provide a basis for evaluating the quality of prone ventilation care for patients with ARDS. Methods Based on the three-dimensional quality structure model,through literature analysis and semi-structured interviews,a preliminary drafting of an ARDS patient prone position ventilation nursing quality evaluation index was established. From September to November 2020,the Delphi method was used to conduct 2 rounds of consultation inquiries to 35 experts to establish the evaluation index of the ARDS patient’s prone position ventilation nursing quality. Results The recovery rates of the 2 rounds of questionnaires were 100%;the expert authority coefficients were 0.85 and 0.89;the Kendall harmony coefficients were 0.155 and 0.122,respectively. The final evaluation index of prone ventilation nursing quality for ARDS patients includes 3 first-level indicators,8 second-level indicators,and 41 third-level indicators. Conclusion The constructed evaluation index of prone ventilation nursing quality for ARDS patients is highly reliable and practical,and can be used as a basis for evaluating the prone ventilation nursing quality of ARDS patients.

Objective To formulate an expert consensus on diaper dermatitis nursing care for infants and toddlers,and to standardize the clinical nursing practice. Methods Literature searching/reviews and expert discussion meetings were used to formulate the main contents of the consensus. The consensus was then revised by 2 rounds of Delphi expert consultations and expert meetings. Results The final expert consensus on diaper dermatitis nursing care for infants and toddlers included 3 aspects of diaper dermatitis nursing care(evaluation,prevention/nursing care,treatment of infection),and a total of 31 items. Conclusion Items in expert consensus on diaper dermatitis nursing care for infants and toddlers are concise and easy to implement,and the consensus is helpful to guide clinical practice of diaper dermatitis nursing care.

Comparison on the changes of tracheal sealing effects of two shapes of direct and indirect measurement procedures with cuffs

Objective To compare the sealing effects of direct and indirect pressure measurement methods on cuffs with taper and columnar shapes. Methods Cuffs were divided into the taper and columnar shapes,both groups were measured using direct and indirect measurement procedures,and the fluid leakage of two groups were observed. The direct measurement method monitored pressure by a directly linked handheld pressure gauge,while the indirect measurement method added a 3-way stopcock between the handheld pressure gauge and the cuff. We locked the 3-stopcock to stabilize handheld pressure gauge reading at 32 cmH2O before measure the pressure. Results The pressure of the taper shaped cuffs in direct measurement group was(21.73±0.30) cmH2O while the pressure of column shaped cuffs was(25.01±0.13) cmH2O,which showed a statistically significant difference (P<0.01). Fluid on the two types of cuffs all leaked after direct measurement. The pressure of the taper shaped cuffs in indirect measurement group was(31.08±0.31) cmH2O,and(31.10±0.17) cmH2O in columnar shape,without significant difference(P>0.05). Fluid on the taper shaped cuffs didn’t leak after indirect measurement procedures yet the fluid on column shaped cuffs all leaked. Conclusion The direct measurement method can cause pressure decreasing in taper and column shaped cuffs as well as fluid leakage. Indirect measurement method can lose only 1~2 cmH2O of pressure for taper and column shaped cuffs,and the taper shaped cuffs can seal fluid leak effectively.
