临床护士营养护理能力量表的编制及信效度检验

Abstract:

Objective To develop the Nutritional Care Competence Scale for Clinical Nurses and to test its reliability and validity. Methods On the basis of literature analysis and semi-structured interviews,the evaluation index system of clinical nurses’ nutritional care competence was constructed through 2 rounds of Delphi expert correspondence,and the pre-test scale was prepared. From December 2020 to February 2021,367 clinical nurses were selected for item analysis and exploratory factor analysis of the pre-test scale. In the second stage,762 clinical nurses from 4 hospitals were selected for confirmatory factor analysis,reliability and validity analysis of the scale to form a formal scale. The competence inventory for Chinese registered nurse(CIRN) was used as the criterion related validity reference. Results The formal Nutritional Care Competence Scale of Clinical Nurses with 5 dimensions and 63 items was formed. 7 common factors were extracted by exploratory factor analysis,and the cumulative variance contribution rate was 75.409%;confirmatory factor analysis showed that the fitting index of the scale structure model was good. The overall content validity index of the scale was 0.968,and the content validity index of each item was 0.833~1;the correlation coefficient between the total score of the scale and the total score of CIRN was 0.788,and the correlation coefficient between each dimension was 0.582~0.809(P<0.05). The overall Cronbach’s α was 0.991;the split half reliability was 0.926,and the test-retest reliability was 0.876(P<0.001). Conclusion The Nutritional Care Competence Scale for Clinical Nurses has good reliability and validity,and it can be used as a tool to evaluate the nutritional care competence of clinical nurses.

摘要:

目的 编制临床护士营养护理能力量表,检验其信度和效度。方法 在文献分析和半结构式访谈的基础上,通过2轮德尔菲专家函询构建临床护士营养护理能力评价指标,并依据此编制预测试量表。2020年12月—2021年2月选取367名临床护士对预测试量表进行项目分析和探索性因子分析,选取4所医院762名临床护士进行量表验证性因子分析及信效度分析,形成正式量表。采用注册护士核心能力测评量表作为效标关联效度。结果 最终形成包括5个维度(营养专业知识、临床营养评估能力、营养健康教育能力、营养支持护理能力、营养护理相关素质)、63个条目的临床护士营养护理能力量表,探索性因子分析提取7个公因子,累计方差贡献率75.409%;验证性因子分析结果显示,量表结构模型的拟合指数良好,量表总体内容效度指数为0.968,各条目内容效度指数为0.833~1;量表总分与注册护士核心能力测评量表总分的相关系数为0.788,各维度与之相关系数为0.582~0.809(均P<0.05)。量表总体Cronbach’s α系数为0.991,分半信度为0.926,重测信度为0.876(P<0.001)。结论 临床护士营养护理能力量表信效度良好,可作为评价临床护士营养护理能力的工具。
