非自杀性自伤青少年自我表露意愿及期待的质性研究

A qualitative study on the self-disclosure willingness and expectation of adolescents’ non-suicidal self-injury behavior

摘要:

目的 深入了解非自杀性自伤青少年自我表露的意愿及期待,为临床护理人员制订预防和干预方案提供参考。 方法 采用目的抽样法,选取2022年2月—5月在浙江省某三级甲等医院儿童青少年心理联合门诊就诊的23例非自杀性自伤青少年作为研究对象,对其进行半结构式访谈,采用Colaizzi 7步分析法归纳并提炼主题。 结果 提炼出4个主题:①有自我表露意愿和行为的青少年,情绪体验存在差异;②有自我表露意愿但没有自我表露行为的青少年,内心矛盾纠结;③没有自我表露意愿及行为的青少年,感受极端化;④非自杀性自伤青少年期待得到表露对象的积极回应。 结论 非自杀性自伤青少年的认知和表露对象的回应会影响青少年自我表露的意愿和行为,青少年对表露对象的回应存在着诸多期待。临床护理人员应提升青少年及相关群体对非自杀性自伤的认知,鼓励青少年自我表露,并给予积极回应。

Abstract:

Objective To deeply understand the willingness and expectation of self-disclosure of non-suicidal self-injury behavior in adolescents,so as to provide reference strategies for prevention and intervention of clinical nurses. Methods Phenomenology in qualitative research was adopted,and 23 adolescents with non-suicidal self-injury behaviors admitted to a combined outpatient clinic of children and adolescents in a hospital in Zhejiang Province from February to May 2022 were interviewed by semi-structured interviews,and the data was analyzed by Colaizzi seven-step method. Results 4 themes are extracted. Theme 1:adolescents with self-disclosure willingness and behavior have diverse emotional experiences. Theme 2:adolescents with self-disclosure willingness but no disclosure behavior have different contradictions. Theme 3:adolescents with no self-disclosure willingness and behavior experience radicalization. Theme 4:adolescents with non-suicidal self-injury expect positive responses from disclosure subjects. Conclusion The cognition of adolescents with non-suicidal self-injury and the response of disclosure subjects play an important role in their self-disclosure willingness,and adolescents have many expectations for the response of disclosure subjects. Clinical nurses should enhance the cognition of adolescents and related groups on non-suicidal self-injury behaviors,encourage adolescents to take the initiative to disclose,and give positive responses.
