1例急性白血病患儿的安宁疗护实践

摘要：

该文总结了运用安宁疗护的理念、知识和技能对1例急性白血病患儿和家长进行安宁疗护的实践经验,护理要点包括：实施呼吸困难、发热的症状护理;通过开展家庭参与的绘画疗法干预、儿童死亡教育对患儿和家长实施死亡教育;召开家庭会议协助家长进行医疗决策;运用音乐和芳香疗法、耳穴疗法缓解患儿家长重度失眠症状;运用心理社会疗法、家庭哀悼、家庭尊严干预、书写穿越时空的信件等干预措施缓解患儿家长的复杂性哀伤。患儿于最后一次入院5 d后舒适、平静病逝,患儿家长的预感性悲伤评估量表评分由逝后7 d的116分降至逝后6个月的76分,取得了较满意的效果。

Abstract:

This article reported hospice care for a child with acute leukemia in the end-of-life stage,including the practical experience of using the concepts knowledge and skills of hospice care and nursing care for the child and his family members. The main points of nursing care include：conducting nursing care for symptoms of breathing difficulties and fever;carrying out family-participated painting therapy interventions,and children’s death education;holding a family meeting;using music,aromatherapy,and auricular acupoint therapy to relieve the insomnia symptoms of the children’s family members;using psychosocial therapy,family mourning,family dignity intervention,and writing letters that travel through time and space to relieve the complex grief of the children’s family members. The score of the Anticipatory Grief Scale dropped from 116 points to 76 points 6 months after the death,which achieved satisfactory results.
